Diagnostic Lab Data: 1/4/21 CT ABDOMEN AND PELVIS WITH IV CONTRAST VESSELS: There is enlargement and complete thrombosis of the left common, internal and external iliac veins, with adjacent fat stranding noted. There is thrombus in the visualized left femoral vein. No thrombus appreciated in the right-sided iliac veins or inferior vena cava. Normal abdominal aorta. IMPRESSION: Extensive occlusive thrombus in the left common, internal and external iliac veins. VAS lower limb venous duplex study, complete bilateral RIGHT LOWER LIMB: No gross evidence of acute or chronic deep vein thrombosis. No evidence of superficial thrombophlebitis noted. Doppler evaluation shows a normal response to augmentation maneuvers. Popliteal, posterior tibial and anterior tibial arterial Doppler waveforms are triphasic. LEFT LOWER LIMB: Slow flow versus acute deep vein thrombosis is noted in the common femoral vein, saphenofemoral junction, and proximal femoral vein. No evidence of superficial thrombophlebitis noted. Doppler evaluation shows a normal response to augmentation maneuvers. Popliteal, posterior tibial and anterior tibial arterial Doppler waveforms are triphasic/biphasic. 1/5/21 LEFT LOWER EXTREMITY AND PELVIC VENOGRAM. MECHANICAL THROMBECTOMY, ANGIOPLASTY, AND STENT IMPRESSION: Impression: Successful mechanical thrombectomy of the iliofemoral thrombus demonstrating a May Thurner syndrome treated with a 16 mm stent and angioplasty

Write-up: Hospital Course: ? Patient is a 43 y.o. female patient who originally presented to the hospital on 1/3/2021 due to Left lower extremity pain and swelling. Patient found to have extensive DVT of left lower extremity and started on heparin drip. Vascular was consulted and recommended thrombolysis. Patient was also seen by IR who took patient for thrombectomy and left iliac stent placement on 01/05/2021. Patient tolerated procedure well. Patient was transitioned from heparin drip to Eliquis upon discharge. Patient given vascular follow-up as well as Hematology follow-up.

Write-up: Day of vaccine, patient started feeling fatigue, malaise, site pain & swelling, joint pain all over, profuse sweating w/no fever, chills, all over inflammation & swelling, hives upper extremity, petechial rash, and the next day after the vaccine, the patient started experiencing swelling in R-axilla (lemon size). 3 days after vaccine all symptoms have resolved except swelling in right axilla.

Write-up: Received Pfizer vaccine, first dose on Wed. 01/13/21 between 12 and 1 P.M. Thurs. 01/14/21 in the afternoon he began to note that he had difficultly walking. Went to bed when he woke up at 5:48 A.M. he reported he had ataxia. Patient reported having to walk in tiny steps to stay upright. He went to the emergency room. Had CT scan of head and found blood clots. MRI performed. Stroke found in right PCA territory, but no loss in strength in left lower extremity. Sensation and vision intact. Strength in all four extremities is 5 out of 5.

Write-up: A petechial rash developed on my bilateral palms of hands, left greater than right, on 01/10/2021. There was no rash noted on my feet or body habitus. When the lesions became darker and more punctate , decision was made to seek medical attention. There was no history of trauma, fever, or skin injury. I saw my Internist, on 01/14/2021 for evaluation and treatment.

Write-up: Allergic Skin Reaction: Moderate-severe pruritic maculo-papular rash present on bilateral hands including 100% finger and palm involvement and ~20% dorsum of hand. Rash extends on anterior arm up to axilla. Rash also present in inguinal area and 100% of buttocks and extends down posterior leg down to knee. Petechiae present bilaterally on ventral hands hand finger tips (likely related to squeezing hands for itch relief) Rash was present around 60 hours after vaccination starting on buttocks and inguinal area and progressed to hands then armpits over the following day. Rash progressing in severity despite 50 mg oral Benadryl q6h, 40 mg oral prednisone daily, and topical hydrocortisone. Rash still present and progressing and no peak severity has been identified yet.

CDC Split Type:

Write-up: Severe Right sided chest pain, right sided muscle spasms and difficulty breathing two weeks after vaccine was administered Diagnosis of bilateral pulmonary embolism was made on presentation to ER. No personal or family history of clots in arteries or deep veins or any risk factors in patient. Received heparin drip, pain medications, muscle relaxants inpatient. Pain progressively improved over days. Was discharged after 6 days on admission. Was discharged on oral anticoagulant (Rivaroxaban aka xarelto)
