Diagnostic Lab Data: 24-Jul-2021: Outpatient emergency care performed urinalysis, EKG, blood work with a focus on troponin levels, and COVID-19 test, and provided IV saline solution and 650 mg Tylenol. Urinalysis: Normal. EKG: Sinus tachycardia. Blood work: Overall normal. Neutrophil levels elevated. Troponin normal. COVID-19 PCR Test: NEGATIVE. 5-Aug-2021: Blood work tests completed: ESR, CBC, WBC, BMP, CRP, and eGFR. Results normal. WBC displayed slightly decreased lymphocytes and slightly elevated monocytes. 6-Aug-2021: CT scan of abdominal/pelvis performed with barium solution and IV contrast (Isovue-370, 80.00 cc). CT results showed no significant abnormalities detected. 23-Aug-2021: Outpatient emergency care performed EKG, heart monitor, orthostatic blood pressure checks, and blood work with a focus on troponin. EKG: Normal Blood pressure: Normal Blood work: Overall normal. Troponin normal. Heart monitor: Normal 25-Aug-2021: EKG performed at primary care visit. EKG displayed occasional PAC, but considered benign and asymptomatic. QT/QTc was normal.

Write-up: 23-Jul-2021: Vaccine administered on upper left arm at 3:00 PM. No allergic reaction. At around 4-6 pm, I felt some sort of mild temperature sensation in the forearms, especially on the left arm. Sensations went away after urinating. At 11:00 PM, I went to bed and shortly after started to have a low-grade fever (100.4 F) accompanied with elevated heart rate (110-120 bpm), headache, and body aches with emphasis at certain locations throughout the night: legs, chest, upper abdominal, arms, lower back. I was able to get up and move around as needed. Took 500 mg Tylenol and lots of water. At 5:00 AM, I walked over to my neighbors apartment and spent the night there in case of any emergency. I hardly slept that night due to the pain. 24-Jul-2021: Symptoms persisted overnight into the next morning, but fever had lessened slightly to about 99.9 F. I became concerned when I still was feeling a high heart rate accompanied with aches near my chest. At 8:30 AM, I went to an outpatient emergency care to checked for any serious issues. Staff performed urinalysis, EKG, blood work w/ a focus on troponin levels, and PCR COVID-19 test, and provided IV saline solution and 650 mg Tylenol. Results came back normal besides an elevated level of neutrophils due to immune response. PCR COVID-19 test showed NEGATIVE. During my visit, aches were lessening, especially the chest/upper abdominal aches. I was discharged at around 2:00 PM and proceeded to get food. Later that night, I experienced dull pain and discomfort in my head and arms and my hands were cold indicating an immune response. I took 200 mg Advil to help alleviate symptoms, but it was an ineffective low-dose. Symptoms subsided sometime in a few hours, but I hardly slept that night too. 25-Jul-2021: Continued to take 500 mg Tylenol and plenty of fluids. Immune response continued to persist at the evening/night time and then eventually subside. Felt nausea and loss of appetite. 26-Jul-2021: Visited urgent care at 7:00 AM and described symptoms to PA. PA checked vitals, which showed normal, and stated that it sounded like a normal vaccine reaction. I later visited a primary care doctor at 4:00 PM. He saw vitals were normal and asked if my symptoms were worsening. I stated that I felt they were slightly getting better, and the doctor noted everything as a normal vaccine reaction. In the evening/nighttime I experienced an immune response again with difficulty in eating. 27-Jul-2021 ? 3-Aug-2021: Difficulties in eating and triggered immune responses persisted. I started taking 30 mg Zinc, 1000 mg Vitamin C, and 5000 IU Vitamin D3 since 7/30/2021. Severity of immune response symptoms are lessening over time and becoming less frequent. Sleep is gradually improving. 4-Aug-2021: Experienced immune response, especially with tender and warm aches in the upper abdominal sometime after consuming a milkshake in the evening. Upper abdominal aches and pain persisted throughout the night with some heartburn. Took Tums which provided mild relief. 5-Aug-2021: Strong loss of appetite resulting in very minimal food consumption with long periods of fasting. Visited primary care doctor and had blood work completed (CBC, WBC, ESR, CRP, and EFGR). Blood work showed normal results with the exception of slightly elevated monocytes and slightly lowered lymphocytes outside of respective reference range. 6-Aug-2021: Visited gastrointestinal (GI) doctor and described symptoms. GI doctor palpated abdominal area, which elicited only discomfort and not pain. He prescribed 40 mg Pepcid. He ordered CT scan of abdominal/pelvic at my request. CT scan was performed with barium solution and IV contrast (Isovue-370, 80.00 cc). CT results showed no significant abnormalities detected. Noticed a slight increase in appetite and lessened severity in immune response. 8-Aug-2021-9-Aug-2021: Slight appetite continued, but small meals were consumed. Mild chest tightness occurred during the evening, especially after moving around at a quickened pace. Diastolic blood pressure was elevated after movement (85-95), but subsided to normal ranges (75-80) when achieving rest. Breathing felt mostly normal. Mostly felt tired and some sort of slight heaviness in the chest. Started taking 40 mg Pepcid. 10-Aug-2021-11-Aug-2021: Mild fatigue and aches in the upper abdominal and chest during the morning. No difficulties in working and moving equipment in hot temperatures. Felt more fatigue and muscle aches the next day. 16-Aug-2021-20-Aug-2021: Took methylprednisolone (4 mg dose pack) 24 mg on first day and had painful hot flashes overnight, primarily on the upper body. Discontinued medication, but still experienced burning nerve pain that diminished with time. 23-Aug-2021-24-Aug-2021: Noticeable return in appetite. Felt like I was back to my usual health prior to vaccination. Experienced heart palpitations with periods of bradycardia (35-40 BPM RHR) mostly in the evening; however, likely related to Sertraline (Zoloft) 25mg that I started on 20-Aug-2021. Visited outpatient emergency care at doctor''s recommendation. Outpatient emergency care performed EKG, heart monitor, orthostatic blood pressure checks, and blood work with a focus on troponin. Results came back normal. 25-Aug-2021: Visited primary care doctor and had EKG performed. EKG displayed occasional PAC, but considered benign and asymptomatic. 30-Aug-2021: Fully recovered from any vaccine side effects.

Basic Metabolic panel, BUN 39 and Creatinine 2.48, eGFR 27. 7/29 CT KUB, (Normal), Referred to Nephrologist . 8/2 Dr ordered Cytology, Urine (Normal) US Renal Kidney, (Kidney''s showed up bright on US.) All other results looked to be normal. 8/4 INR ordered (Elevated INR at 3.6) Reduced INR to 2.3 on 8/9 Urinalysis still showing blood in urine and elevated BUN and Creatinine , 2.4 on 8/19, 8/23 Urinalysis still showing blood in urine and elevated BUN and Creatinine. 2.0 INR on 8/26.

Pain down both legs. Write-up: Light-headed and slightly shaky approximately 5 minutes after receiving vaccine, so once I got my card we left to go immediately to the Emergency Room, approximately 10 minutes away. The symptoms progressed including strong tremors, mucus production, coughing and loss of bladder. I administered an EpiPen in the vehicle. I was immediately triaged at hospital and admitted for observation. No additional medications were administered. I was under observation for four (4) hours and discharged, making sure I had additional EpiPens. Since vaccination, I have been extremely reactive in a Mast Cell Flare, and am taking 50 mg of Prednisone, Benadryl and Nebulizer as needed.

On 14-AUG-2021, the patient experienced feeling cold due to the thin blood due to the Eliquis treatment. Write-up: Patient developed a rash about 3 days following vaccination. It was maculopapular and was treated with steroids. Patient developed progressive dyspnea shortly after the rash that was treated with oral Ceftin, oral Augmentin, and steroids. This progressed to the point where she was hospitalized on August 10, 2021. Her oxygen needs were very significant requiring HFNC and bipap. She was never intubated. She was ultimately diagnosed with cryptogenic organizing pneumonia and has since responded to therapy.

Write-up: I had laparoscopic cholecystectomy 6/28/2021 and presented to my PCP with shortness of breath and dyspnea. On July 6, 2021, I was diagnosed with findings on CT scan revealing extensive embolic material in the subseqmental and segmental branches of all 5 lobes as well at the bifurcation of the right main pulmonary artery. There was a small infiltrate in the left lower lobe which may likely be developing pulmonary infarct. There was also embolus present in the left lower lobe pulmonary artery with extension into multiple seqmental and subsegmental branches. Since starting blood thinner I have not had any breathing issues. Will need to take blood thinner for remainder of my lifetime.

Write-up: Vaccine received on August 18th, and side effects started two days later. Patient reports to be as healthy as a horse and was swimming like the young people. Body started aching and patient started to measure her body temperature. The temperature kept on increasing until it got to 102 or 103, where she then started to take Dayquil/Nyquil. The temperature decreased to 100 with this, then it would increase. After, patient tried taking 400mg Ibuprofen every 6 hours but it would also increase/decrease the temperature the same. Write-up: I started to receive an abnormal feeling in my body near my heart. I had palpitations where my heart rate would jump from 60 bpm to 100 bpm. This has never occurred before and I believe this is a result of the Janssen vaccine. I had drank plenty of water the days after the vaccine, and I was not anxious about anything. These palpitations occurred in the middle of the night. Feeling that this was abnormal, I rushed myself to the nearest emergency room I was given a few lab tests (basic metabolic panel, complete blood cell count, troponin I). Two electrocardiograms were also performed. The nurses provided one liter of lactacted ringer during my stay at the hospital. I was discharged and left with a bill where over was requested to be paid by the patient.

Initial information was processed along with the additional information received on 30-AUG-2021. The patient''s height, and weight were not reported. The patient''s concurrent conditions included high blood pressure and was on CPAP (Continuous positive airway pressure ) machine, non-alcohol user, and non-smoker. The patient had no known allergies and no drug abuse or illicit drug use. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 207A21A, and expiry: 21-SEP-2021) dose was not reported, 1 in total administered to left arm on 30-APR-2021 for prophylactic vaccination. No concomitant medications were reported. On 27-JUL-2021, the patient developed shortness of breath and fever (Suspected covid -19 infection) and was taken to emergency room. There patient tested positive for covid-19 infection (suspected clinical vaccination failure). The patient was transferred to other hospital and was on CPAP machine along with Bi-PAP. The patient had low blood oxygen. The patient was put on oxgyen. On unspecified date the patient become negative for covid-19 infection. The patient had internal bleeding and was on blood thinner. The patient had hematoma and could not breath. On 19-AUG-2021 the patient was again transferred to another hospital and was intubated which made him worse. On 27-AUG-2021 the patient passed away. Write-up: STROKE; This spontaneous report received from a consumer concerned a 56 year old male with unknown race and ethnicity. The patient''s height, and weight were not reported. The patient''s past medical history included: gall bladder removed recently (June 2021) due to upper Gastrointestinal (GI) issues, and concurrent conditions included: diabetic, blind, hyperlipidemia, and high blood pressure. The patient experienced drug intolerance and makes sick when treated with hydrocodone. The patient received covid-19 vaccine (suspension for injection, route of admin not reported, and batch number: 202A21A expiry: unknown) dose was not reported, 1 total, administered on 16-AUG-2021 to left arm for prophylactic vaccination. No concomitant medications were reported. On 20-AUG-2021 Friday, the patient was fine after receiving the vaccine, he even mowed the yard. On 21-AUG-2021 Saturday, at the morning patient woke up and could barely get out of bed and could not walk due to his feet being numb, then reported his hands were numb. On 23-AUG-2021 Monday, patient went to the hospital and received fluids and other care, and was discharged. On 24-AUG-2021, patient went to his health care provider (HCP), who sent him to the hospital via ambulance to be admitted through the emergency room (ER) for possible stroke, although reporter stated they doubted that diagnosis due to both sides of the body being affected. On 24-AUG-2021, the patient was admitted to the hospital and has had a spinal tap, Computer tomography (CT) scans, Magnetic resonance imaging (MRI), and other tests. As per the reporter patient were treated it as Guillain Barre Syndrome(GBS). The reporter was upset the vaccine facility gave the sheets with possible side effects to them after having received the vaccine. On 29-AUG-2021, reporter reported that patient was still in the hospital and that the physicians state they was treating it as Guillain Barre Syndrome and/or a stroke. The reporter stated that, patient was treated with shots in the stomach as well as intravenous (IV) medications possibly Immunoglobulins but reporter does not exactly know the names as currently the hospital was not allowing any visitors and so only form of communication with patient through phone. The reporter called back to update the diagnosis, which she stated hospital found high protein in the spinal fluid which could be markers for stroke. As per the reporter physician called it an unusual stroke being that it was close to the brain stem. The reporter repeated that she did not like the fact that the vaccine administrators provided the information about the vaccine after the shot was given and that not that it would have changed our minds at that moment but still maybe something that they could do to improve the vaccination process. The patient''s weight was 149 pounds, and height was 64 inches. The patient''s past medical history included: vertigo (rarely), and concurrent conditions included: hypertension, non alcoholic, smoker, and smokes marijuana, and other pre-existing medical conditions included: The patient was otherwise healthy. The patient experienced dry cough when treated with lisinopril. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 1808986, expiry: UNKNOWN) dose was not reported, 1 total, administered on 27-JUL-2021 to left arm for prophylactic vaccination. Concomitant medications included diazepam, losartan potassium, and valaciclovir hydrochloride. On 27-JUL-2021, right after vaccination, the patient experienced a bit of sore arm, which lasted a couple of days. The patient could not remember exact date for rest of report, but it was started possibly around on 15-AUG-2021 or 16-AUG-2021. The patient''s feet started feeling strange, like pins and needles, and she was in a lot of pain. The patient had diarrhea three times. She went to the chiropractor because her back was bothering her. The patient could not endure the pain (started in the feet, then to legs, then to hips, and to upper back), so they went to Emergency room (ER) in the middle of the night. The ER gave her some pain medication (unspecified) and she left (not admitted). It was reported that two days later she was back in the ER, and was admitted for 9 days. Write-up: Parenthesis began middle of the night the night of vaccine (felt like body was on fire) The buzzing/parenthesis still persists everyday and it has been 6 months. Write-up: Patient started experiencing lightheadedness 30 seconds after injection. We had him sit down and at that time he became less responsive and his eyes rolled back in his head but he remained upright but not very responsive. Write-up: 5 minutes after receiving the vaccine, the patient felt dizzy. She was sitting upright in a chair at the time. She passed out and had a small seizure. We moved her to the floor and her breathing became very shallow. We adminstered an epi-pen at 1:41 pm and called 911. We took her BP using an automatic cuff at 1:45 and it was 96/64. We handed her care over to EMT at 1:50 pm. They transported her to ER because her lactic acid was very high.

Write-up: Janssen COVID-19 Vaccine EUA: COVID-19 case resulting in Hospitalization / Death. Patient received Janssen Vaccine on 03/13/2021. On 8/6/2021, pt presented to ED with increasing diffuse abdominal pain that started 3 days prior. Pt reports he was diagnosed with COVID-19 on 8/5/21 and had increasing shortness of breath with cough and headache which began 8/1/21. CT showed acute uncomplicated appendicitis and confirmed patchy ground-glass opacities within the bilateral lung bases consistent with COVID pneumonia. He was given 1 g of Tylenol, 1 g of IV Rocephin, 6 mg of Decadron, 500 mg of IV Flagyl, 2 doses of 4 mg of morphine, 4 mg of Zofran and 40 mg of IV Protonix in the ED. Pt also received remdesivir. He developled hypoxia on HFNC 50/50 sating 84-87%. pO2 49. Patient was able to maintain sats in the lower 90''s CXR showed worsening pulmonary edema and he was given 20mg IV lasix. Patient transferred to MICU service for worsening respiratory status on 8/13. Patient was intubated and started on proning protocol. He continued to require pressors and sedation with propofol, versed, and dilaudid. Palliative was consulted and discussed with family comfort care. Patient expired on 8/24/21.

Write-up: I pretreated with 50 mg of Benadryl and Tylenol at 12:15. I got the injection at 01:41pm and at approx. 02:20 pm on my way home I developed hives and my throat started close up. I was taken to the ER. I was given .3 of epinephrine and then 60 mg of prednisone and 50 mg of Benadryl. Shortly after, about an hour later, my throat closed up again and I could not breathe. I was given an additional .3 of epi and more Benadryl and an IV was put in. I was discharged that evening with a 10mg taper of prednisone and 2 epi pens, I was directed to take Benadryl every 4-6 hours as needed. I have also had joint and muscle pain, a fever, sore arm. I am still itching, mostly in my face and left palm and left bottom of my foot. I am unable to work, will have to stay home for a week to make sure this is resolved.

Fluids were administered MRI was conducted Benadryl was advised MRI showed no signs of stroke. Benadryl helped the stiff neck subside. Stamina and blood pressure still frequently bothersome.

Write-up: Serious issues with blood pressure stability disease blackouts, serious headache initially, my headache subsided three days later, Extreme stiff neck and joints of hands, some light sensitivity, and feet relieved primarily by Benadryl. Write-up: The pain started in the injection site and lingered for about 3 days. The Friday after my vaccination, the pain moved from my left shoulder muscle down to my elbow. It was almost a dull throbbing pain on 4/30/21. This lasted through the weekend the next week until finally, the pain moved down to my left wrist. It hurts every time make a fist, bump it slightly, or jam it just doing normal things. It has been hurting for nearly 5 months now and I never had any wrist problems prior to the vaccine. Write-up: The next day I got a fever just over 100 degrees which lasted for about 12 hours accompanied by a headache and chills. After the fever and chills went away the headache persisted. It would go away and come back but never went away for long. It enough to be very annoying on the pain scale and was not a normal headache. After 4 or 5 days the headache had not gone away, and I had gotten info about side effects from the J& J vaccine and decided to call my doctor because I have a preexisting blood clotting condition. My doctor sent me to the Hospital on 4/23/2021 and at that time I still had the headache. She wanted me to go to confirm I did not have a blood clot. The y ordered a CT scan of my head, and they checked my platelet count. After the hospital visit the headache lasted for an additional 4 to 5 days.

Write-up: This is ongoing, still running tests, but it is appearing to be Parsonage-Turner Syndrome. I Had a herniated disc, but that was a side issue. I started having back/shoulder pain about two and a half weeks later, woke up at 3:00am in excruciating pain. Went to the emergency room the next day, we were out of town. Drove home, went to chiropractor, GP Doctor, had an MRI, surgery for herniated disc, but symptoms keep escalating. Have lost all use of my right hand and arm, absolute no nerve ability to use my hand, extreme weakness right arm, extreme and ongoing pain.

Write-up: Six minutes in I felt flushing and heart pounding and some swelling in my throat. I had pre-medicated with benadryl so I took another one. The heart pounding eased in just a minute or two and also the flushing. I had had an allergic reaction to Moderna January 7th , so thought I was ok to just go home and take the same meds the ER had given me in January. I called my doctor''s office the next day to get more prednisone.

Write-up: Tinnitus in both ears on and off the first 4 weeks after vaccination. Persistent tinnitus in both ears about a month after the shot and ongoing. The first few months I had some tingly-feeling of nerves in my left arm. So far I''ve visited an urgent care to make sure my ears were okay. Then followed up with an ENT for about 5 visits until the doctor said nothing was wrong from what he could see. Prescribed a drug for help sleeping.

Write-up: Patient passed out 5-10 minutes after receiving vaccine. 911 was notified. His wife said he "got up to walk to the car because he overheated and fainted upon standing." He lost consciousness for a second time. The patient laid on a couch in the pharmacy until the ambulance arrived. His wife noted that he "has a history of heat related syncope and has been very stressed this week." Vaccine given - 5:45pm, Syncope - 5:50
