Write-up: This is ongoing, still running tests, but it is appearing to be Parsonage-Turner Syndrome. I Had a herniated disc, but that was a side issue. I started having back/shoulder pain about two and a half weeks later, woke up at 3:00am in excruciating pain. Went to the emergency room the next day, we were out of town. Drove home, went to chiropractor, GP Doctor, had an MRI, surgery for herniated disc, but symptoms keep escalating. Have lost all use of my right hand and arm, absolute no nerve ability to use my hand, extreme weakness right arm, extreme and ongoing pain.

Write-up: Six minutes in I felt flushing and heart pounding and some swelling in my throat. I had pre-medicated with benadryl so I took another one. The heart pounding eased in just a minute or two and also the flushing. I had had an allergic reaction to Moderna January 7th , so thought I was ok to just go home and take the same meds the ER had given me in January. I called my doctor''s office the next day to get more prednisone.

Write-up: Tinnitus in both ears on and off the first 4 weeks after vaccination. Persistent tinnitus in both ears about a month after the shot and ongoing. The first few months I had some tingly-feeling of nerves in my left arm. So far I''ve visited an urgent care to make sure my ears were okay. Then followed up with an ENT for about 5 visits until the doctor said nothing was wrong from what he could see. Prescribed a drug for help sleeping.

Write-up: Patient was seen in the Clinic on May 14, 2021 and reported that she was having pain and swelling in her injection site from her Covid-19 Johnson and Johnson vaccine that was given on April 3, 2021 Clinic. Vitals at that time were BP: 118/78, Left Arm, Pulse: 96, Temp: 97.7 F, Weight: 164 lbs, Oximetry: 97%, BMI: 26.47 kg/m2. She was having swelling in her hands and feet as well and also was having difficulty arising from a seated position. Initial information was processed along with the additional information received on 27-AUG-2021 The patient''s height, and weight were not reported. The patient''s concurrent conditions included: diabetes, hypertension, and atrial fibrillation. The patient''s height, and weight were not reported. The patient''s concurrent conditions included: idiopathic chest cavity pain, allergy to contrast dye, no alcohol use, and non-smoker, and other pre-existing medical conditions included: The Patient had history of no drug abuse or illicit drug usage. The patient was not pregnant at the time of report. The patient experienced drug allergy when treated with phenytoin (Dilantin). The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported) dose was not reported, 1 total administered to left arm on 18-MAY-2021 for prophylactic vaccination. The batch number was not reported and has been requested. The patient was on unspecified medications for idiopathic chest cavity pain. On 18-MAY-2021, day of vaccination the patient took a nap and woke from severe lower back pain, the pain was really bad that she couldn''t get up and could barely walk which lasted for about 3 days. The patient mentioned that she will be on blood thinners for the rest of her life. At the time of report, the patient still has chest pain but just not as severe. Write-up: Pt was transported to the ED 9/2/21 for hypoxia and hypotension per her report. She actually denied shortness of breath or cough but reported throat pain for the last few days which she attributed to thrush. Per EMS report, BP was 70/40, and SPO2 mid 80% on 2L. She received 1.5 L IV fluid in ED which corrected her blood pressure and was placed on NRB initially then weaned to HFNC but became hypoxic and was placed back on NRB. Her ABG showed hypercapnia. Pt required admission to ICU.

Write-up: 30 days after Janssen Injection, patient felt ill, dizzy. Noticed large quantity of small red blood dots on legs, thighs, lower extremities. Small bruise on left hand and left elbow blossomed in to massive black and blue and brown bruise. Patient went to Well Center on 8/28/2021. Doctors immediately send patient to Emergency Center. Dr. took blood test and determined that patient was in critical condition with blood platelet count below <3k where normal platelet count should be 150k to 400k. Patient Emergency evacuated via Ambulance Hospital main facility bypassing Emergency room and going directly in to critical Care unit. Dr. took another blood sample and verified Platelets and red blood cell counts extremely low. A platelet IV was administered to prevent patient from crashing. Patient moved to Critical Care Unit upstairs where teams of doctors started determining action plan for recovery and initial cause of incident. Patient blood tested for COVID, came back negative and count was normal. Dr.''s tested Kidneys, Liver, Spleen, lower abdomen for any irregularities and found none. Then performed a Bone Marrow Needle biopsy with a MRI for needle placement. Bone Marrow test came back Normal. Patient stayed in hospital until 9/1/2021 with a platelet count of 159k. Patient given 30 day pill supply of predniSone 20mg 3 times per day for 30 days. Massive bruising of hands, elbow, legs. Looked like patient had been in car accident.

Write-up: The evening of 8/27 the muscle and joint pain began. Early morning of 8/28 fever, muscle and joint pain, my skin hurt to touch, and it felt like all of my nerves were sore, headache. Evening of the 28th most of the symptoms has subsided. Morning of the 29th I woke up was numbness and tingling in my left arm/hand. Just thought I slept weird and brushed it off. Morning of the 30th I woke with numbness and tingling in left arm and leg. By the time I got to work it had spread to my face. I reported the symptoms to my coworker who recommended I see employee health. I saw employee health who told me I needed a head CT ASAP and walked me to the ED at work. The ED ran the CT, labs and a urine test and then I met with Dr. Dr. said he didn?t think it was a vaccine reaction and was going to consult neurology as my CT was clear. He returned and stated that the neurologist said they have seen a few cases like this and it is a vaccine reaction. To my best recollection it was described as a neurological inflammatory reaction. An MRI was ordered to rule out a CVA, Dr. noted that the neurologist said all the MRIs they had seen so far with this had been negative. I returned from MRI and another Dr. had taken over. He said my MRI was clear and that neurology said my symptoms should go away on their own within a few weeks and I had increased numbness or a headache to return to the ED. 9/3 still have the numbness and tingling in my left side. It makes it difficult to work from the weakness and brain fog. I get tired very easily.

Write-up: Patient presented to the pharmacy to receive her COVID-19 Johnson and Johnson vaccine. After vaccination we asked the patient to remain in the pharmacy to monitor. Shortly after the patient experienced syncope and fell of the chair which they were sitting on for us to monitor them. I asked a fellow staff member to call 911. I and another staff member placed the patient back on the seat from which she fell to better assess the patient. she returned to consciousness. She still had her mask on and said for a second she was having difficulty breathing. I went to grab an Epipen to administer. when I came back the patient was coherent enough to ask that I do not administer the Epipen. I asked if she had difficulty breathing to which she responded "may I just take my mask off". I responded "of course do you need assistance?". she stated "no I got it". the patient then took off their mask and repeated, "Do not give me the Epipen I can breathe fine I just need a second". Upon further evaluation the patient did not exhibit swelling of the face, nor any redness. they also stated that they were able to breathe adequately so we refrained from administering the Epipen. I counseled the patient to focus on breathing techniques and closely monitored the patient. We remained with the patient until Emergency services arrived. The patient was then taken to the hospital.

Write-up: Within 5 minutes of receiving vaccine iIstarted to feel light headed then nausea. As it started to worsen, I notified staff of my symptoms. At this time I passed out. Within several seconds, I started to gradually regain consciousness. I felt very weak and still lightheaded and nauseated. Paramedics arrived, my BP was 60/40. I went to the hospital for monitoring. I felt much better within a couple of hours. All vitals were back to normal by 8pm and I was discharged from the hospital. The next 2 days I experienced mild flu-like symptoms including fatigue, nausea and muscle aches. My arm where the shot was given was sore for 6 days before the pain started to subside.

Write-up: Complained of throat itchiness approximately 17 minutes after vaccination was administered. Seconds later, started having sensation of throat tightness and began coughing. Onsite clinician administered 0.3 mg epi pen to left outer thigh at 10:16 am. Patient refused 911 and Ambulance. She left vaccination site at 10:19 am and was transported to nearest Emergency Room by her friend who is an EMT. Patient left before vitals could be collected. Write-up: 2 hours after receiving vaccination was very lightheaded and slightly disoriented. Had slight issue with gait. Went to bed early to sleep off thinking it was anxiety. Woke up the following day feeling normal. Starting at 10A shortness of breath began by noon I had a severe headache, shortness of breath, chest pain, shooting pain in my lower arms, thighs and calves, bad back pain, chest tightness, abdominal pain, low grade fever and heart palpitations. Was admitted to the ER at 6PM on 8/25. Was treated with electrolytes, breathing treatment x-rays and blood work. Chest X-ray was clear, blood work was in normal ranges except for lymph auto level being low (12%). It was determined that inflammation was causing the shortness of breath and a course of prednisone was prescribed but was advised to be taken only if not improving in a few days. I avoided taking the prednisone due to its effect on the vaccine. I instead used 6 puffs of Proair HFA into a spacer in substitution for a home nebulizer four times a day and continue to at this time (9/3/2021). Over the course of 2 more days many of the side effects subsided. However, I still fight with shortness of breath, random shooting pains throughout my body, head pressure, occasional abdominal pain/cramps and occasional heart palpitations. I returned to the doctor today, 9/3 for a check up on the ongoing issues and was given a script for an x-ray and blood work. This time the X-Ray has revealed an opacity in the lower right lobe of my lung. I was given a Z-Pak script while awaiting blood work results and have to return for a 6 week X-Ray appointment.

Write-up: Had no issues until i went to bed. Early morning on 9/2.. I was shivering so intensely that my muscles hurt despite a 72 degree temperature in he house. Write-up: BELLS PALSY; This spontaneous report received from a health care professional concerned a 21 year old male. The patient''s weight was 200 pounds, and height was 72 inches. As patient''s medical history, patient''s blood pressure was elevated when he went to the hospital, patient had been a non-smoker and drinks alcohol (As socially, patient would drink his beer). The patient had no known allergies and did not have any drug abuse or illicit drug use. The patient had no known medication regimen. Patient''s mother has diabetes. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, and batch number: 1808982 expiry: UNKNOWN) dose was not reported, 1 total, administered on 10-AUG-2021 for prophylactic vaccination. No concomitant medications were reported. On 31-AUG-2021, the patient had numbness and tingling on the right side of his face with pain behind his right ear. It worsened through the day and later in the evening he was sent to the local hospital. Upon arrival at the hospital he was diagnosed with Bell''s Palsy (G51.0). Patient had a neuro check on the patient and his Glasgow Coma Scale/Score (GCS) score for eye opening was 4, verbal was 5, with right sided numbness. Write-up: Received J&J 8/6/2021. Fever, fatigue, ears ringing, foggy brain, within 12 hours. This continued until Wednesday 8/11/2021 at which time I felt a tightness in my chest, pinching and continued ringing ears. I also continued feeling foggy brain. I called my primary care doctor and reported that by blood pressure was 160s over 90s and she prescribed blood pressure medication as I had previously had a medication induced uncontrolled hypertension in 2019/2020. Saturday 8/14/2021, I went to a walk in clinic with continued pressure in my chest and had an EKG which was normal. She referred me to the ER for a cardiac workup. I went to the ER and had an EKG, bloodwork and CT with contrast. All test results came back normal. One month post vaccine, continued chest pressure, ringing in the ears, and foggy brain. Additionally, within three weeks of the vaccine, I have come down with a UTI, infected hair follicle in my armpit and an eczema episode has occurred on both my eyelids causing them to be red, swollen, itchy, oozing, and painful.

Diagnostic Lab Data: I went to my former GP 4/13 who gave me a covid test and it was negative. On 4/13 I did blood work to rule out a clot due to the leg pain and bloodwork was normal. I went to the ER on 4/16 and they ran blood and, did a CT, lung xray, covid test and all was normal. I was getting sicker so my dr told me to go back to the ER on 4/20 and they ran additional blood, CT, EKG, and found no problems. They thought I was having an intense immune response to the vaccine and prescribed prednesone for 5 days which did not help. I also saw a rheumatologist who referred me to a neurologist. Both ran blood work. My ANA is high as are markers of inflammation. I swithed to the medical system and saw a new GP on May 18. At another appointment they decided to admit me, and I was hospitalized June 16-June 18 because the doctors were concerned about the worsening muscle weakness. In the hospital I had multiple blood tests, 2 MRIs and a pulmonary function test. Again the only abnormal result seems to point to autoimmune disorder. After the hospitalization I saw a rheumatologist on August 13 who thinks I may have Sjogren''s syndrome (I have experienced dry mouth and eyes even before the vaccine, but the vaccine exacerbated these symptoms). I''m sorry I may not have included all the dates of my appointments, there have been many. I am happy to give access to my medical records. I have not gotten a diagnosis to explain most of my symptoms.

Write-up: I got the vaccine on 3/31 and had normal side effects for 2 days-headache, nausea, fatigue. The side effects went away. Then on April 4, about 4 days post Vaccine I began having extreme pain in my legs. At about a week and a half post-vaccine I began having all over body illness-- I broke out in a rash on my abdomen, extreme fatigue, blurred vision, night sweats, insomnia, headaches, body and joint aches, and my most alarming symptom was weakness in my legs. Within a few weeks the weakness spread and affected my pelvic floor, abs, arms, chest, and face. I was in bed for basically 6 weeks other than Dr appts. In addition to muscle weakness and pain my neurological symptoms included strange sensations like muscle buzzing, twitching, and tremors, and hands and feet going numb when I am lying down. I have never experienced any neurological problems before the vaccine. Walking up and down stairs was especially difficult. I have seen some improvement in my symptoms with time and rest but fatigue and some other neurological symptoms remain. I have not been able to work full time since my symptoms started.

Write-up: The day after the vaccine I began experiencing leg pain in my right leg, as the day went on I began to have veins in the same leg puff up and stick out. The leg pain got worse over the week after my vaccine. on the 5th day after vaccine my entire body went numb and it felt as though I could not breathe and was rushed to the ER via ambulance. A week after I received the vaccine I am now experiencing arm pain and muscle weakness in my right arm along with occasional throbbing in both of my arms with continued pain in my right leg and some in my left leg as well. Write-up: Directly after the injection my chest got heavy, I had shortness of breath, dizziness, I could not move my arms then I lost consciousness. I woke up on the floor with people over me. They tried to get me back up and I lost consciousness again. The ambulance was called. Emergency ambulance assessed that my heart rate was extremely low in the 40''s. My blood sugar was also low. I was kept for observation for 2 hours. Write-up: Received the vaccine 27Aug2021 and 8 hours later felt chills, nausea, body aches, and severe headaches which limited me to just the bed for 48 hours. I took some tylenol which cured most symptoms and was able to do normal activities after 72 hours on 30Aug2021. Then the headaches came back within the next 24 hours with a throbbing pressure at the base of my cranium towards the rear, the temple areas, and sometimes radiating throughout my jaw. I would take tylenol and it would temporarily relieve the pain, but would eventually come back. I recognized that there was a problem and on 3Sept2021, I talked to the COVID19 Information hotline and they directed me to see a doctor. That same day I went to a walk-in clinic and the Doctor recommend I go to the ER to rule out any blood clots. I then proceeded to the ER and received an IV cocktail - Benadryl, Compazine, Toradol. The Doctor took some blood, urine, and a conducted a CT Scan of my head. My vital signs were normal and the Doctor discharged me saying that there was no indication of blood clots from the CT Scan. I still continue to have the headaches at least one a day which is remedied by tylenol.

Write-up: Hidradenitis Suppurativa. It started on my left arm pit with 2 bumps. Got very infected and had drained at urgent care thinking it was just ingrown hairs. Then 2 weeks later, I got 5 more on the same arm pit. I got one on my right arm pit now and one on my groin. I have taken Doxycycline HYC 100mg 3 different times for infections. Write-up: PARALYZED/FROZEN TOES; SCAR ON KNEE STARTED SWELLING; SCAR ON KNEE PAINFUL; FOOT PAIN; TORN MENISCUS; FALL; TIGHTENING OF THE JAW/ PARALYZED/ FROZEN JAW; SWOLLEN ARM/ FOOT SWELLING; TENDER ARM; This spontaneous report received from a patient concerned a 57 year old White female. The patient''s weight was 169 pounds, and height was 65 inches. The patient''s past medical history included: scalp psoriasis, colon cancer, 18 inches of transverse colon removed, depression, lymph nodes removed (due to prior colon cancer surgery), abscess tonsil, freezing of jaw, fall, and past injuries on the left side, and concurrent conditions included: non-smoker, non-alcoholic, and scar on knee. The patient had no history of drug abuse or illicit drug usage. The patient was previously treated with citalopram for depression, bupropion hydrochloride for depression, omeprazole for depression; and experienced muscle spasms when treated with procaine hydrochloride for drug used for unknown indication, and drug allergy when treated with epinephrine. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 1805020, expiry: UNKNOWN) dose was not reported, 1 total, administered on right arm on 05-MAR-2021 at afternoon around 16:00 for prophylactic vaccination. No concomitant medications were reported. On 08-MAR-2021, the patient experienced a small amount of swelling underneath the arm that was a little tender that started for 3-4 days and only lasted a couple of days. On 08-APR-2021, the patient started experiencing a tightening of the jaw, similar to temporomandibular pain. On 21-APR-2021, the patient''s jaw was paralyzed/frozen. The patient could only fit a finger in the jaw. The patient visited emergency room on same day for the jaw and was not admitted. The scar on knee from the fall 15 years ago started swelling (was not too painful at the time and also given as knee pain and swelling). Write-up: 9/2/21: The patient is a 30 year old female who presents to the ED for evaluation of strokelike symptoms. At approximately 11 PM tonight the patient reports having sudden onset of confusion and difficulty speaking. Patient and family explain she is having difficulty getting words out and has slurred speech. She also reports experiencing left-sided weakness and numbness. A short while after also started to experience dizziness and if she tries to focus on something she sees double. Denies any associated headache or neck pain. No chest pain or dyspnea. No fever or recent illness. No recent trauma. Patient states she received the J&J vaccine 2 days ago, denies any other new medications. Denies similar episode 30-year-old acute female here for evaluation of strokelike symptoms. Patient evaluated on arrival as a stroke alert. Last known normal was approximately 11 PM. Neurologic deficits include slurred speech, left-sided numbness and weakness, dizziness and double vision. On exam, the patient is noted to have decreased sensation to the left face/arm/leg as well as decreased grip strength and left lower extremity weakness. NIHSS 4. Patient transferred emergently to CT which returned negative for acute intracranial hemorrhage. Case discussed with teleneurology who also evaluated patient. Decision made to treat with TPA. Patient was monitored closely post TPA infusion and on serial neurologic exams was noted to have significant improvement in her speech, sensory deficits and strength testing. NIHSS improved to 2. Toxic/metabolic work-up overall returned reassuring. Patient to be admitted to ICU for further care and evaluation 9/4/21 Cardiology: 1. Acute neurological event resolving after TPA administration with subsequent large PFO found on transthoracic echocardiogram. Treatment recommendations regarding her PFO ultimately revolve upon what neurology feels etiology of her neurologic event was. If it was felt it was ischemic in nature treatment options include aspirin and Plavix for 3 weeks followed by a long-term low-dose aspirin versus PFO closure. Numerous randomized trials PFO closures only shown to be beneficial in patients with minimal atherosclerotic risk factors, large PFO''s and positive neuroimaging. With a negative diffusion-weighted MRI I do not believe there is any indication for closure. If it is felt that her neurologic event is a complication of the Covid vaccine no further management of her PFO is indicated. If it is felt that it was a true ischemic event despite her negative diffusion-weighted MRI then would recommend dual antiplatelet therapy for 3 weeks followed by low-dose aspirin indefinitely. Appreciate the opportunity to participate in patient''s care. Please call with any questions. 9/4/21: DISCHARGE 9/5/21: READMIT 30-year-old female with depression, anxiety, and GERD presents to the ED with recurrent neurological symptoms. She reports the onset 1 hour prior to arrival of difficulty speaking, left-sided weakness. She had a similar presentation 3 days ago and actually received intravenous TPA with improvement in her symptoms. Her evaluation included a normal head CT. She had an MRI/MRA of the head, MRA neck, and MRV, all of which were normal. 9/5/21 ICU: This is a 30 year old female who was recently hospitalized for expressive aphasia and left sided weakness several days after after receiving the J&J COVID-19 vaccine, and subsequently was given TPA and sent to the ICU. MRI of the brain, MRA of the head/neck, and MRV of the head were unremarkable at that time. TTE demonstrated a patent foramen ovale, however cardiology was consulted and there was no indication for closure. Patient symptoms returned to normal and she was discharged from the hospital on 9/4/2021. She returned to the hospital today complaining of aphasia and left-sided weakness. Teleneurology was consulted in the ED and recommended imaging as well as admission to observation for possible LP. MRI brain and MRV head were negative for acute intracranial abnormality; however, there were several small punctate foci of increased hyperintensity within the white matter which were nonspecific findings. On presentation, she is afebrile with temperature 98.5 ?F, heart rate ranged 80-132 bpm, RR 16, BP 147/86 mmHg, SPO2 100% on room air. On exam she is laying flat on her back in bed, no acute distress, pleasant. Flat affect. Tired appearing. Alert and oriented x3. CN II through XII grossly intact bilaterally. Strength 5/5 in all extremities. Speech is slowed but not dysarthric. Sensation is intact throughout except for mild numbness in the lateral aspect of her left leg. Heart is regular rate and rhythm, distal extremities are warm and well perfused. Lungs are clear to auscultation, breathing comfortably on room air. Abdomen is soft, bowel sounds present, nondistended. 9/5/21 Neurology: Impression: Transient speech change and L sided weakness/numbness Recommendations: 1) LP - cell count differential protein glucose cytology cultures, lyme ACE, VDRL 2) EEG 3) Check ANA, SSA/SSB, Complement, Anti-TPO, autoimmune panel 4) Can be discharged home if initial results negative with close outpatient follow up. 9/6/21 Hospitalist: Radiology call - showing "moderate to large epidural hematoma within the posterior epidural space with hemorrhagic fluid seen from the level of L1 extending caudally to the L5 level with the most significant portion of the hemorrhage located at L2-L3 causing severe central canal stenosis at L2-L3." 9/6/21: The patient is a pleasant 30 y.o female with severe back pain and left leg paresthesias with a large epidural hematoma from L1 to L5 with severe stenosis at L2/3 causing lumbar radiculopathy. I discussed with her the options including conservative care versus surgical intervention. Given the patients significant symptoms with severe stenosis at L2/3, I recommended surgical intervention which would entail a L2 and L3 laminectomy and evacuation of epidural hematoma. I discussed with her the potential risk, complications and lack of guarantees. Pt wishes to proceed with surgery. The Intensivist discussed with Hematology who agrees that the tPA should be out of her system. Pt had PT, PTT and INR today that are normal. The LP was performed at the L2/3 level and I may need to place a patch and glue if there is any CSF detected. We will proceed with surgery urgently.

Write-up: Patient is a 51 yr old male with a PMHx significant for HTN, HLD, and bilateral uveitis who presented to ED after testing positive for COVID19 with worsening symptoms of chills, malaise, and shortness of breath. Patient states he tested positive for COVID 14 days ago. He continue to experience worsening symptoms so he called his PCP on 08/19 and was prescribed methylprednisone and a zpack. Pt states he took it for 2 days however his symptoms persisted so pt presented to the ED at another hospital system/ on 08/22 where a chest xray showed COVID19 pneumonia. Pt was prescribed Pulmicort and scheduled for convalescent plasma on 02/25. He was not admitted as he was noted to have a sat of 99% and was tolerating room air at the time. He presented to receive his convalescent plasma today. Patient states during his treatment he continued to be short of breath and his O2 sats dropped to the 90s. Patient was brought to the ED via EMS. On admission he was found to an elevated CRP and ferritin. His white count was stable and he was afebrile. His chest x-ray showed bibasilar patchy airspace opacities favoring infectious etiology. Patient states he was vaccinated with Johnson & Johnson on 03/2121. Patient is currently being admitted for further care.

Write-up: On July 20th about lunch time I started with a fever chills, aches and I had her take me to urgent care I took a rapid test it was negative. I continued to have a fever that move 101-103. I took medication to lower my fever it never went away. On Sunday 25th I didn''t have a fever I still didn''t feel well. On Monday the 26th I was going to go to work, I started vomiting uncontrollably and I rode in ambulance. I could not breathe or walk, I was so weak. They tested me again test came back positive for Covid. I did the monoclonal infusion. I was in low 80s for my oxygen and they gave me supplemental oxygen which brought it up to high 80s and low 90s. They did not have a bed said someone would have to die or go home they called near by hospital. No beds available so they sent me home with supplemental oxygen and monitored my oxygen that was set up to my smart home. I''m still really weak and fatigued, exhausted and shortness of breath. I''m getting better just really slow.

Write-up: Within 25 minutes of administration itching started in injection arm and moved to whole body. Benadryl 50mg taken PO X2 with 4 hours, no relief. Layed down and felt throat constricting so went to ER first time. Rec''d benadryl 50 for third dose, prednisone 60, pepcid, and went home with an epi pen. Symptoms worsened 2 days later and returned to ER with difficulty swallowing and facial and neck swelling. Dizzy for 1 week after, inability to concentrate, neck pain.

Write-up: This is a 55-year-old male with PMH of HTN, DM, CHF, COPD - dependent O2 at 2L/nc , atrial fibrillation, obstructive sleep apnea on CPAP, lupus, bipolar disorder, chronic pain, osteoarthritis, hypothyroid who presents to ED with complaints of worsening shortness of breath associated with productive cough with white phlegm and headaches x 2 days. Denies fever or chills. Reports generalized body aches/malaise. Denies abdominal pain, nausea or vomiting. Was hypoxic with O2 sat of 78% on RA upon EMS arrival; oxygen saturation has improved to 94 when place with oxygen inhalation.

PROCEDURES PERFORMED None. CONSULTATIONS 1. Bone Marrow Transplant team. 2. Infectious Disease HOSPITAL COURSE The patient is a very pleasant 28-year-old male with past medical history as listed above including history of CML, status post allo stem cell transplant in October 2019; graft-versus-host disease; recent status epilepticus requiring ventilator for airway protection; and now newly diagnosed COVID-19, who presents to Emergency Department secondary to worsening symptoms. The patient was seen in the Emergency Department on August 29 with symptoms of fatigue, nausea, vomiting, and cough. COVID-19 was positive at that time. Blood cultures were obtained. The patient was discharged home. He returned the following day with worsening symptoms. It was also noted that a single blood culture was positive for coag-negative staph. With the COVID positivity, the patient did receive Regeneron antibodies on August 29. Dex was not started as the patient was not hypoxic. He was medically stable with stable vital signs and was admitted and started on IV fluid hydration. Blood cultures were redrawn and he was started on empiric vancomycin. The coagulase-negative staph positive blood culture was felt likely to represent contamination, but in the setting of immunocompromised state and COVID infection, Infectious Disease was consulted. They agreed this was likely contaminant. Oral vancomycin was discontinued. Repeat blood cultures remained negative at 48 hours. ID recommended starting remdesivir on 08/31 given he was less than 7 days into symptoms and his cycle threshold is on the lower side. Chest x-ray was obtained and was negative for acute process. The patient remained very stable and had clinical improvement and was felt medically stable for discharge on 09/01/2021. He will receive a second dose of remdesivir prior to discharge. Given immunocompromised state, the patient may discontinue quarantine respiratory isolation on September 15. The Bone Marrow Transplant team was consulted and will arrange outpatient followup.

Write-up: August 11th - J&J covid vaccine August 25th - not feeling well late evening better next morning, bilateral legs hurt, slight numbness in arms, legs and lips August 26th - not feeling well again late evening, bilateral legs hurt more than on the 25th, numbness in arms, legs and lips. Took 3 Tylenol for pain to sleep August 27th - continue symptoms from the prior day August 28th - bilateral leg pain sufficient not to be able to sleep. Went to Emergency room. Doctor ran blood test and ultrasound of bilateral legs. Doctor stated no acute reason for bilateral leg pain August 29th - felt fair, still have pain and numbness in bilateral legs and arms August 30th - Called in sick at work. Went to another ER and took Emergency room blood test records. ER doctor performed lower back CT scan, prescribed steroid pills, tramadol, and steroid injection August 31st - Went to spine doctor, discussed treatments including physical therapy and steroid injection, felt fair, unable to sleep due to continue bilateral leg numbness and pain September 1st - Went to another ER - ER doctor reviewed medical records, prescribed muscle relaxant Flexeril - medication did not help and did not sleep well September 2nd - went to Medical Center and discussed treatment, had minor back adjustment, started taking steroid pills September 3rd - Went to Neurologist - saw Dr. and was diagnosed with Guillain-Barre and treatment to follow with findings of impaired walking and left foot drop and ataxia September 4th - could walk with difficulty September 5th - went to assigned hospital per Dr. since not able to walk by myself - received first dose of IVIG at 11 pm - plan for IVIG treatment for total of 5 days September 6th - walking with walker and second round of IVIG at 2137

Write-up: Given vaccine @ 14:15 and taken to observation area, no acute issues reported. ~14:25 pt asked for water, c/o temporary LOC. Pt laid down and feet elevated, LOC less than 7secs. Pt very diaphoretic, cool and pale. Pt reports feeling very hot and clammy. EMS called and arrived within 10mins. HR 100, SBP 115s. Pt continued to improve over time, reporting feeling hot but less lightheaded. Upon arrival of EMS pt HR still in the low 100s and SBP 112, no abnormal rhythm found. EMS advised to take pt to ED for observation d/t reports of feeling dizzy and lightheaded. Pt reports he has passed out from blood draws before and did not eat lunch prior to arriving for vaccination.

Write-up: I experienced blood clots and DBTs in both lungs (pulmonary umbilications) which caused me to spend 5 days in the hospital (06/13/2021 - 06/18/2021). The hospital tried Xarelto but I ended up GI bleed out of my bottom (where blood should not come from). CTs and X-rays were performed on 06/13/2021 and was released while on the the blood thinner. I was admitted to another hospital where they did a series of tests (X=rays and CTs everyday). A tube was inserted into my lungs to drain the mucus and I ended up an infection on the outside layer of my lungs and my left lung ended up collapsing. I still have the drainage tube in my lungs until the doctor can get me where I need to be before surgery can performed on my lungs (needs to gain weight before the procedure can be performed). I am still experiencing pain as well.

Write-up: High blood pressure/heart rate, chest tightness, light headed, dizziness. Went to the Hospital. Was released as GERD. Immediately following I started having deep pains in my arms and legs, extreme hot flashes, numbness in my right arm, and both hands. My GP saw me two weeks after the shot. Ran blood work and sent me for ultrasounds on my arms and legs, negative for DVT. Symptoms continued to worsen, now left elbow will not straighten and right shoulder has issues. Knees are always painful and my hands are extremely painful when I first wake and at different times of the day and it''s difficult to walk upon initially standing. I have to have chiropractic care, and have massages every 2-3 weeks to keep moving because my muscles are tight and all my joints are painful.

Write-up: TRANSVERSE MYELITIS; BELL''S PALSY; CHILLS; NAUSEA; This spontaneous report was received from literature: SARS-CoV-2 Vaccination-Induced Transverse Myelitis. 2021 Jul 25; 13(7): E16624. This report concerned a 44 year old female. The patient''s height, and weight were not reported. The patient had no past history. The patient''s pre-existing medical conditions included: Her family history was unremarkable for muscular disorders, multiple sclerosis, stroke and any rheumatological disorders. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of administration, batch number: Unknown and expiry date: Unknown) dose, start therapy date were not reported, 1 total for prophylactic vaccination. The batch number was not reported and has been requested. No concomitant medications were reported. The patient was presented to the emergency department with a three-days history of worsening back pain associated with nausea and urinary retention. She also reported numbness and weakness in the lower extremities for the last two days along with low-grade fever, chills, and body aches for two to three days. On physical examination, blood pressure was 135/83 mmHg, heart rate of 78 beats per minute, temperature of 99.1 Fahrenheit, respiratory rate of 11 breaths per minute and oxygen saturation of 99% on room air. She was awake, alert, attentive, and oriented to time and person and did not have any nuchal rigidity, meningismus, or Lhermitte''s sign. Her cranial nerves II-XII examination was grossly intact. She had normal muscle tone and strength in both upper and lower extremities with normal coordination and gait. Exaggerated (3+) deep tendon reflexes were noted in bilateral upper and lower extremities and positive Babinski signs were present bilaterally. Sensory examination was abnormal with decreased vibration in bilateral toes, and mild paresthesia in the neck and abdomen. The rest of the physical examination was unremarkable. Initial laboratory analysis showed an unremarkable complete metabolic profile, complete blood count, and a negative SARS-CoV-2 nucleic acid amplification test. The patient underwent magnetic resonance imaging (MRI) of the cervical, thoracic spine, and lumbar spine with and without contrast, showing increased signal throughout the spinal cord extending from the C2-3 segment into the upper thoracic spine (Transverse myelitis). She underwent MRI brain with and without contrast, which was unremarkable for any inflammatory process. Neurology services evaluated her and recommended starting high-dose methylprednisolone therapy for the next three days. She required multiple straight catheterizations due to urinary retention. The next day she had significant improvement in her symptoms. Lumbar puncture was performed with cerebrospinal fluid (CSF) analysis showing a white blood cell count of 227 ?/L, red blood cells of 25 ?/L, total cell count of 100 with 96% of lymphocytes, 3% of monocytes, and 1% of eosinophils. CSF chemistry analysis showed glucose of 71 mg/dL, protein of 43 mg/dL, albumin of 0.6 g/dL, and lactate dehydrogenase of 8 units/L. The rest of the workup, including the venereal disease research laboratory test, Toxoplasma gondii antibody, neuromyelitis optica IgG antibody, antinuclear antibody screen and cryptococcal antigen, were negative. CSF bacterial, viral, and fungal cultures were sent and were later reported as negative. The myelin basic protein was noted to be 2.8 mcg/L, and the (Immunoglobulin G) IgG index was 0.67. No oligoclonal bands were present. The CSF pathology showed small lymphocytes and rare monocytes. To prevent recurrence or exacerbation of myelopathy, neurology services decided to start her on plasma exchange for five treatments over 10 days after the completion of three days of pulse dose of methylprednisolone. While undergoing plasma exchange, she developed Bell''s palsy, suggesting the persistence of the underlying inflammatory process despite three days of methylprednisolone and four sessions of plasma exchange. Repeat MRI cervical and thoracic spine with and without contrast was performed after five sessions of plasma exchange and showed a decrease in signal intensities in the cervical and thoracic spine depicting improvement in her condition. Treatment medications (dates unspecified) included: methylprednisolone. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. The patient recovered from bell''s palsy, chills, and nausea, and was recovering from transverse myelitis.
