Write-up: Approximately a week ago on 9/7/21 patient tested positive for COVID-19 back then she was having URI like symptoms which patient reports are getting better. Patient has been undergoing chemotherapy for lymphoma and did receive her last chemotherapy on 9/3/21. For past few months patient has been having recurrent urinary retention. Currently on chronic foley, last urine culture was positive for Pseudomonas outpatient treatment with Keflex. Patient has some symptoms of abdominal discomfort. Patient reports at baseline she is able to stand using walker and ambulated. She has history of chronic compression fractures of vertebra. She has metastasis of lymphoma to her vertebrae. She denies fevers chills. Denies vomiting. Denies any focal neurological deficit. Denies breathing discomfort chest pain palpitation. Denies abdominal pain constipation. Patient reports improving symptoms, no difficulty in breathing, on room air. Admitted on 9/15/21. Radiological finding could be lagging clinical resolution or improvement. At the same time patient might get worse we need to observe. We will not start patient on steroids remdesivir. Continue to monitor clinically. Incentive spirometry. Discharged 9/20/2021

Write-up: Changes in my menstrual cycle- I have had an IUD since 2017 and haven''t had a period since insertion in April 2017. Once I received the vaccine my periods started again and have become increasingly worse; my period has become painful and heavy. Write-up: Soreness in injection sight, shortness of breath the following morning, fatigue, 1 week and 2 days after injection began having symptoms of covid scratchy throat mild cough. Write-up: DISSEMINATED INTRAVASCULAR COAGULATION; VACCINE-INDUCED THROMBOTIC THROMBOCYTOPENIA; THROMBOCYTOPENIA; DEEP VEIN THROMBOSIS; BILATERAL PULMONARY EMBOLISM; HEART WAS ENLARGED; CYST OF SOME SORT; BALANCE WAS STILL A BIT OFF; DECREASED APPETITE; MALAISE; This spontaneous report received from a consumer concerned a male of unspecified age, race and ethnicity. The patient''s height, and weight were not reported. The patient''s pre-existing medical conditions included: The patient had no previous underlining or pre-existing health conditions. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: Unknown, expiry :unknown) dose was not reported, start therapy date were not reported, 1 total, administered for prophylactic vaccination. The batch number was not reported. Per procedure, no follow-up will be requested for this case. No concomitant medications were reported. It was reported that the vaccine resulted in a severe reaction, placing the patient in a life-threatening position. Approximately 6-7 days after receiving the vaccine with having no previous underlining or pre-existing health conditions his symptoms worsened. He was having severe issues. Malaise set in, decreased appetite, dry cough along with other symptoms. The patient went to emergency department (ED) and was discharged home with unspecified antibiotics, an unspecified antispasmodic & was negative for corona virus disease (COVID). He went home for 2 days after spending the night in the ER. No signs of improvement, he just progressively getting worse and had bilateral lower extremity edema setting in. On 08-AUG-2021 he went back to the ED and was hospitalized back again after nothing had improved. Though he did not feel well another concern was the prolonged sitting in the same spot not moving about or getting up, he could very well develop a deep vein thrombosis (DVT), not to mention anemia. He went back to the ED and was hospitalized back again on 08-AUG-2021 after nothing had improved. His labs were drawn and his platelet levels had dropped to a critical value needing a platelet transfusion. Most of the labs were within normal range (WNR), however his platelets barely increased causing concern, thus dropping down again. The chest computerized tomogram (CT) was remarkable for bilateral pulmonary embolisms as well as an ultrasound (US) on his lower extremities, which confirmed a DVT in one of his legs. Specialists were consulted due the concern that his body was attacking & destroying the platelets that had been infused yet it was baffling to all that most of his other labs were "okay". His hemoglobin and hematocrit levels did drop but not enough to need a transfusion despite the prolonged bleeding. It was concluded that he was indeed suffering from vaccine induced thrombotic thrombocytopenia with symptomatic disseminated intravascular coagulation. Two rounds of IVIG (immunoglobulin) were given along with low dose anticoagulants to help dissolve the clots. He had been in the hospital sitting in the ICU with a foley catheter, unspecified intravenous (IV''s), oxygen, some leg compression device and a ton of leads stuck to his body being monitored. He had multiple CT scans, multiple ultrasounds, an echocardiogram, an unspecified transfusion, lab work done daily. He had blood clots in his leg and in both lungs, he was short on breath and tired. Never he had been so sick. It turns out that everything he was going through was a severe reaction to the COVID19 vaccine. He was in the hospital living through those concerns right now. After spending 6 days in the intensive care unit (ICU)/ progressive care unit (PCU) they were able to get him discharged and bring him home to continue the current regimen he was on while hospitalized. Treatments would continue for months. The battle continued, but they were hopeful. Since his platelets have increased significantly after receiving 2 rounds of IVIG & most of the other labs were WNR, and other diagnostic studies were unremarkable. They still need to complete the Magnetic Resonance Venography (MRV) scan as an out-patient. He regained a good amount of his strength; he was not 100% but he could get up and move around and he had graduated from a walker to a cane. He had to use the cane as his balance was still a bit off and he could not afford to fall since he was on unspecified blood thinners. He still had blood clots in both of his lungs and his leg. The clots in his lungs make it difficult to breath and he had a cough that comes on quick if he talked too much. He got winded and tired very quickly if he move around too much and he needed to sit back down. His heart was also enlarged due to the extra work it has to do pushing blood through partially blocked vessels in his lungs. The effect was that he felt really cyst and his heart began to race so again he need to sit and rest. He also had a new lump appear on his leg that he was unsure as to what it was, initial concern was another clot. He had an ultrasound done to check it and he was awaiting the results. The doctors after further inspection thought it might be a cyst of some sort. He had multiple appointments with specialists over the next few weeks as follow-ups to his hospital stay. In the meantime, he had to wait for his body to break down the clots which he was told would take months. Treatment medications (dates unspecified) included: platelets, unspecified blood thinners, anticoagulants, and IVIG and his treatment included foley catheter, unspecified intravenous (IV''s), oxygen, some leg compression device and a ton of leads stuck to his body being monitored. The patient''s height, and weight were not reported. No past medical history or concurrent conditions were reported. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 206A21A, and expiry: 21-SEP-2021) dose was not reported, administered on 13-SEP-2021 for prophylactic vaccination. No concomitant medications were reported. On SEP-2021, the patient experienced dizzy and almost fell and laid down. On 13-SEP-2021, the patient experienced tingling in right hand. On 14-SEP-2021, the patient experienced cold. On 14-SEP-2021, the patient experienced pain in back of legs at every joint at back of knees, hip and ankles. On 14-SEP-2021, the patient experienced chills. On 14-SEP-2021, the patient experienced headache with pain in front of head when bends over. On 14-SEP-2021, the patient experienced fever of 100.5f. On 15-SEP-2021, the patient experienced pain in both eyes. On 15-SEP-2021, the patient experienced sweating continuously. On 15-SEP-2021, the patient experienced tingling in face. Laboratory data included: Body temperature (NR: not provided) 100.5 F. On 16-SEP-2021, the patient experienced pain on right side of face from eye to the chin. Write-up: Client reports around 11:00pm of the day he received the vaccine he developed a "blood clot" at the left AC area. He called Health Department on 9/15/21 at 10:22am to report the incident. He had tried to call his doctor that morning and did not get in touch with them. Health Department staff encouraged him to go to the urgent care. Client went to facility on 9/15/21. A left upper extremity venous doppler ultrasound was performed and the impression was "No deep vein thrombus in the left upper extremity." Write-up: So July 16th I went to the clinic because I had allergy symptoms that were not responding to typical remedies for 4 days I was also experiencing a low grade fever. As I was pulling up to the building I suddenly noticed I lost my sense of smell. Felt like it went off like a light switch. I went in and they tested me for COVID-19 they did rapid and the longer one and both came back positive. Later I realized later I also lost my sense if taste. From then until July 28th my symptoms were low grade fever, hot and cold chills, fatigue, and missing a lot of work (working from home), I needed to take a lot of naps. Loss of taste and smell gradually started coming back beginning July 26th. July 27th I started to feel and then that night, I woke up in the middle of the night with a severe headache. I took Tylenol and waited 4hrs and took ibuprofen and nothing helped. When I woke up I woke up with a migraine. So, I called the clinic and made an appt. He prescribed some medication for me that helped. The medication was Fioricet 50-325-40mg (combo of 3 drugs) . I had migraines for for a week and then headaches for a week. I woke up with headaches for the entire month of August. On the 29th I had another telehealth appt with Doctor because they were still really intense. He said the only way to make sure nothing was going on was to go to the ER and get a brain scan. So after you spoke with him I went to the ER. At the ER they did a CT scan and gave me a headache/migraine cocktail. The cocktail only worked for that night. They said to follow up with my primary doctor. I followed up with Dr. and let her know I was still having headaches even after the pain medication and the migraine cocktail so she prescribed me something else. She prescribed Mobic 15mg daily for 15 days. I took the medication as prescribed and now I am fine.

Write-up: I had a bad cough, but I could not cough anything up. I was really tired. I had no aplite. I was confused as well. I had a fever. When I did cough up something it was black and gray. The Rheumatoid doctor - Diagnosed me with Fibromyalgia - never had issues with that before this happened and also diagnosed with Osteo Arthritis to the Knee. Write-up: First symptom - I started itching and my lips started swelling up. I took a Zyrtec right away. that night, I had severe chills. It was worse than after birth chills. The next day;, 18th, I had severe body aches and I couldn''t touch my body because everything hurt. After that day, I noticed tingling and numbing in finger and toe - joint pains in finger and toe. I had pain between my joints on my fingers. I also had pain in my wrist joints (both) and that radiated up to my elbows. Feet were tingling and numbing. I called my doctor each of these days. Doctor did try to give me Prednisone to help with inflammation. About a week or week and a half after. It (Prednisone) inflamed all my joints and made the pain worse - knees, hips, bottom of my feet, shoulders and where the injection site was I had a lot more pain. I had sensitivity to cold on my feet. There are times when my feet are super cold. Every day I would have a different joint pain - it rotates joint to joint - I''m still having that. I don''t know from day to day which joint it is going to affect. But I''m still having the tingling in my toes and the tingling and the puffiness in my fingers. Two weeks ago, I started having jaw pain and teeth sensitivity. It was really difficult to chew on food. Also got a sensitivity to cold in my mouth. Those newer symptoms lasted for about a week to a week and a half steady but now it comes and goes. Every time that I have these symptoms, I have migraines, too. And I haven''t migraines for a long while before this started happening lately. I am being seen by workmen''s comp doctor and a Holistic doctor. Urgent Care visit at one point - this visit was towards the beginning and was right after I took the Prednisone. They told me to see my doctor - didn''t help me too much.

Write-up: The patient presented to the - and was vaccinated with a dose of the Johnson & Johnson (Janssen) vaccine. Approximately 5 minutes after administration, the patient called for the nurse. The nurse immediately ran over to the patient and the patient stated "i am feeling light-headed". The patient then slumped in her seat. A request for EMS was initiated. Write-up: 20 minutes post vaccination - sweating Later in day began itching, with rash, followed by wheezing, self treated at home. By following morning reports hands curled under, eyes swollen, severe headache, skin was "on fire", full body spasms and cramping. Seen in ED, treated with muscle relaxant, and Percocet. Went on to have reported aphasia, left side numbness of hand and foot, left hand tremors and random muscle spasms. Also began stabbing chest pain radiating to jaw. Write-up: I was having a lot of weird pain and a headache. It kept getting worse and worse. I went to the ER. They did not know what was going on. They said it could not be a stroke. They gave me Aspirin and another medicine but I do not remember which one. They did an MRI, which came back normal. They think I was having an anxiety attack but we do not know. Recently like 3-4 weeks ago it felt like I was getting my period, it was odd. I felt very sick. I had diarrhea. I also had vaginal bleeding. I was really sick for a few days. Write-up: Approximately 30 minutes after receiving the vaccine, I experienced shortness of breath, dizziness, tingling of my arms and fingers and could feel my heart racing. I called 911 and an ambulance came to a parking area just a short drive from the pharmacy where I received my vaccine. I was checked out by the paramedics. My blood pressure was elevated, my heart rate was elevated. My oxygen saturation was only 92. I chose not to be taken to the hospital because my symptoms began to improve after some time had passed. I attempted to drive home shortly after and had to pull over and call a family member to come get me as the dizziness had returned.

The patient''s height, and weight were not reported. The patient''s pre-existing medical conditions included: The patient had no change in lifestyle and had no allergies. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 1808982, expiry: 21-SEP-2021) dose was not reported, 1 total, administered on 16-SEP-2021 to left arm for prophylactic vaccination. No concomitant medications were reported. On 16-SEP-2021, about an hour or two, patient started to feel yucky. On the same day, patient had convulsions and flu like symptoms throughout the day with a fever brought to the emergency room (ER) at 12:45 AM on 17-SEP-2021 with a temperature 102.4 Degrees Fahrenheit at the triage with severe back pain, vomiting and convulsing. The patient was administered Motrin, Phenergren and IV fluids. The blood works were performed, fast covid-19 screening, CPK and comprehensive metabolic panel. The patient was discharged at 3 AM and returned home. It was reported that flu like symptoms body ache, fever, back ache, nausea, vomiting and convulsing which got progressively worse as the day wore on. The patient''s concurrent conditions included hematoma right thigh, hypertension, atrial fibrillation (A-fib), gastrooesophageal reflux disease (GERD), restless leg syndrome, vitamin D deficiency, depression with anxiety, and chronic hyponatremia. The patient experienced throat swelling when treated with corticotropin. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, intramuscular, batch number: 105031, expiry: Unknown) dose was not reported, 1 total, administered on 10-MAR-2021 to right arm for prophylactic vaccination. Concomitant medications included amiodarone, apixaban, calcium carbonate, carvedilol, colecalciferol, diclofenac diethylamine, famotidine, fluticasone, furosemide, and paracetamol. On 10-JUL-2021, the patient presented to emergency department with hip pain (arthralgia) of two weeks in duration, and underwent computerized tomogram (CT) angiogram showed a right inguinal hematoma and active extravasation and was medically managed. The patient subsequently developed shortness of breath (dyspnoea) and pneumonia and treated with 7 days of cefepime. on 30-JUL-2021, patient tested sars-cov-2 test positive for COVID-19 (suspected clinical vaccination failure) which was the anticipated day of transfer to skilled nursing facility. On 04-AUG-2021, the patient experienced abdominal pain prompting kidney, ureter and bladder (KUB) (Urinary system X-ray) which showed small bowel obstruction (SBO) but patient deferred NG tube. Later patient had CT chest, neck and abdomen which showed worsening of pneumonia with aspiration of bowel content and SBO and possible oropharyngeal perforation. Patient did agree after detailed discussion with family about CT findings and interventional radiology (IR) able to place NG tube but shortly after she began throwing up bile (vomiting) and needing high flow oxygen. The patient was not able to maintain oxygen saturation. The hospitalization date was not reported. The patient was hospitalized for 18 days. Write-up: Around 9:30 pm on 9/21/2021 (36 hours after receiving the vaccine), my top lip started to swell. The swelling then moved to my left eye and eventually the entire left-side of my face was swollen. We headed to the ER and once we got there, I started to get hives all over the left-side of my body. A few hives appeared on the right-side of my neck and down my right arm. The ER doctor gave me IV Benadryl, Pepcid and a steroid. It is now 10 am on 9/22/2021 and my lips, left-side of face and left eye are still swollen.

Write-up: clinically dx''d splenic vein thrombosis based on below data/presentation: He developed Covid infection July 2020 19 March 2021 Johnson & Johnson Covid vaccine History of splenomegaly, assoc w/ pain, 17.2 cm on ultrasound 28 April 2021 up from 12.7 cm noted on 15 March 2021, it had been normal sized at 12.9 cm 8 January 2020 History of anemia dating back to 2011, with hemoglobin of 12.2 and MCV of 86 October 2020 History of thrombocytopenia dating back to 2011 with platelet count of 128 October 2020 Referral to hematology July 2021, denies chronic use of heparin or quinine or significant amounts of antibiotics in his adult life or Tylenol or NSAIDs or excess walnuts though he does like to eat nuts daily and does not take any medicinal teas of note he mentions a 3 inch clot in his spleen when he was a teenager that required an approximate 10-day stay with IV heparin, he declined splenectomy due to his history of multiple surgeries for mitral and aortic valve replacement, initially with tissue valve at 3 months old followed by mechanical valves first at 3 years old and then have been repeated as needed and he is followed by a coumadin clinic with a goal INR of 2.5-3.5 per report, also he has a history of juvenile rheumatoid arthritis, he has received blood transfusions when he had bleeding ulcers and he has never had his Coumadin goal changed that he can recall based on the spleen clot when he was younger or the GI bleed in the past related to bleeding ulcers which I also believe was when he was young, he has had a subdural hematoma in the past that I believe was spontaneous, he has had tubular adenomas noted on colonoscopy in October 2020 and he has had an upper endoscopy in 2015 Prior labs have shown immature platelet fraction normal in 2015, anticardiolipin antibodies negative in December 2011, normal white blood cell count, CMP normal in May 2021, HIV negative in 2015, hepatitis ABC panel negative in 2014, homocystine elevated at 16.2, TSH normal in March 2021, peripheral smear review normal in May 2021 per report, IgA had been elevated in 2011 but normal in 2013, he does not drink much alcohol at all, has had an ANA positive greater than 1280 but double-stranded DNA negative, he does have a family history of clots including a sister who had to give up OCPs and a grandmother who had sudden death which may have been related to a clot, he does not have any history of known malignancy or HLH or lupus or sarcoid or amyloid and has not used G-CSF and does not have known liver disease Labs July 2021 showed white count 6.6, hemoglobin 14.9, platelet count 163, MCV 87, hepatitis ABC panel negative, copper normal, TSH normal, EBV negative, HIV negative, ferritin 74, TIBC elevated, iron saturation 14%, reticulocyte count 1.5%, folate normal, B12 428, kappa lambda ratio normal, PNH flow symmetry negative, fibrinogen 349, PTT 52, JAK2 with NexGen sequencing negative, BCR able negative, SPEP negative for M spike, RF and CCP antibodies negative 24 August 2021 ultrasound of spleen with Dopplers showed spleen now decreased back to 12.2 cm, mild heterogeneous normal sized liver with normal flow

Write-up: About 5 minutes post-vaccination, patient fainted. He came to within seconds, but reported memory loss between the time that he sat and the time that he fainted. He was sweaty, pale, and shaking. He reported feeling nauseated which he contributed to not eating breakfast yet. About 3 minutes later, he reported tingling in his hands. A few minutes after that, he reported the same tingling in his feet. EMS was called when he fainted. They assessed him and decided to take him to the hospital due to him still feeling poorly. The patient stopped by about 2.5 hours later to report he was ok. I left him a message the next day to check on him, but never heard back.

Write-up: I experienced exhaustion at work the following week, started losing weight, muscle fatigue/weakness, extreme headaches , dizziness, and has to go to the hospital on 09/12/2021 and they prescribed me a muscle relaxer which help relieve the chest pains. Write-up: Having Stage 4 cancer and having Alzheimer''s, the patient did not leave the house, except for appointments. He had an appointment with the oncologist on 07/22/2021. And on 07/29/2021 he had lab work at Lab Corp. On 08/02/2021 he was complaining of headaches . The paramedics took him to the ER and he was given an MRI -nothing was abnormal. The ER doctors stated that everything looked good. On 08/04/2021, I called the paramedics again because at 02:00 AM, I heard disturbance and I went into his room and he was disoriented and distressed. The paramedics took him to the ER and he was admitted into the hospital, Hospital on 08/04/2021. On 08/04/2021, he had a fever of 103. On 08/06/2021 he was tested for COVID-19. And on 08/07/2021 I went to the hospital to visit him and they told me no visitors. I called the hospital later that day and they told me he tested positive for COVID-19 on 08/07/2021. He was released from the hospital on 08/21/2021. I have no choice but to believe he contracted COVID-19 in the hospital because he did not leave the house.

Write-up: Pt received COVID-19 vaccination, when he began to stand, vaccination nurse noticed that he became very pale. Nurse sat him on chair with back against the wall. Pt asked nurse for water and juice. Pt became very anxious and extremely diaphoretic (dripping), given ice pack and towels - Clinical Lead RN called. 1631 - Clinical Lead RN called to room and observed pt to be very pale and extremely diaphoretic. Attempted to take radial pulse from left arm, pulse too thready, irregular and weak to read. Apical pulse was irregular and weak in 30''s. B/P 60/32 Left arm. RN grabbed emergency kit to get Epinephrine. 1632 - Epinephrine given in Right Thigh. Pt appears confused and states he is scared and "doesn''t know what is happening". 911 called and Paramedics dispatched. 1636 - While Vaccination nurse continues to dry and talk to the patient, Lead RN continues to auscultate apical pulse, which slowly begins to get stronger, up to 70 bpm. (pt states that he is worried and asks if we are being honest with him, will he be alright). Pt states that the only med that he takes is Adderall and that he has no history of this happening before with other injections. 1640 - Color is slowly returning to patient. Apical pulse is now up to 90 bpm and is strong and regular. B/P in left arm is 112/62. Paramedics have just arrived and will be taking pt to Emergency. Pt able to stand and is much more alert but seems a little in shock over what has just happened. Pt able to stand and walk to gurney for transport.

Diagnostic Lab Data: Emergency room doctors tested for infection and found none. They also administered an ECG and results were normal. March 26 approximate date. I was referred back to my PCP who tested for thyroid function, iron levels, diabetes and all results normal.

Write-up: Fever, flu-like fatigue, chills, bursts of pain throughout head and body, swollen gums. These symptoms subsided within a few days. About a week afterwards symptoms of numbness, weakness and tingling in extremities began along with bouts of dizziness and extreme fatigue. Symptoms progressively worsened for about 6 weeks and then began to improve. Fatigue is the only persistent symptom and it is improving. Emergency room doctors tested for infection and found none. They also administered an ECG and results were normal. I was referred back to my PCP who tested for thyroid function, iron levels, diabetes and all results normal.

Write-up: I was vaccinated on 5/26. in mid-June I experienced swelling behind my left knee and started developing spots/sores on the front of the shin. The swelling did reduce after a few weeks but the spots persisted. The week of July12 my calf began to swell and walking was painful. I went to the clinic on July 19 and the hospital later that morning. Substantial blood clots were found in my entire left leg. Genetic tests for coagulation prevalence were negative. No other co-morbidities for blood clots exist.

Write-up: Initial symptoms within the first 2 days were severe headache and low-grade fever, fatigue. The symptoms gradually faded, with the exception of the fatigue, which lingered. 5 days after the injection I experienced a sudden heaviness of limbs, a strange sensation in my gums and roof of mouth, and then a wave of faintness in my head. I called 911 because I thought I was going down. The paramedics concluded that I did not have an allergic reaction, and stayed until my vitals returned to close to normal. 6 days after the injection I experienced two more such episodes, each to a lesser degree. Write-up: August 2021 I had got covid with the normal symptoms like diarrhea, nasal drainage, low grade fever, itchy eyes, and a headache but I didn''t lose my sense of smell this time like I did when I had got covid last year in May 2020. Write-up: Three days after the vaccine I started to have extreme fatigue. One week after that I started to have heart palpitations and I called my doctor. I went to the ER and I had ST elevations in my Stemi. I was in the hospital one night. I was given a Heparin drip and aspirin. I was discharged with an unexplained ST elevation and MI. Diagnostic Lab Data: CT brain 1. Moderate atrophic and small vessel ischemic changes of the brain without CT evidence of an acute intracranial process. 2. Bilateral mastoid effusions or mastoiditis. MRI brain 1. Acute stroke of the left posterior parafalcine region. 2. Multiple areas of hypodensity on susceptibility weighted imaging in the cerebellum likely representing hemosiderin deposition from prior microvascular hemorrhage. 3. Bilateral mastoid air cell effusions. 4. Chronic microvascular ischemic changes.

Write-up: Patient had vaccine in March 2021, had acute CVA in September 2021. It is my medical opinion that this is not related to the Janssen COVID-19 vaccine because the stroke is remote from the vaccine administration, and in a patent with previous history of strokes, hypertension, and diabetes.

Write-up: I started having difficulty breathing in mid April. In May I started getting more sick. I could not breath and I had SOB. Coughing on and off. Headaches. There were times where I could not even get up and I was coughing so much. I could not be at work. That went on and off. It went away for 2-3 weeks. On 06/30/2021 I went down hill. I started getting sick again. It felt like a normal cold. That cold turned into bronchitis. The cough lasted for about 3 weeks. I kept getting tested for Covid and it was always negative. I went to urgent care and they said I had bronchitis. They have me inhalers because it was so hard for me to breath and not cough. I had asthmatic like symptoms. I started getting better after using inhalers. I still use them till this day. I went into surgery because my appendix was about to erupt. I had surgery in the begging of August and they released me after surgery. I was still coughing. The pain was worse because of the cough. Now I am on and off coughing till this day. I started having headaches, fever, chills, kept coughing. I thought I had Covid again. I went and got tested and it came back negative, I still have that cough and SOB.

Write-up: Patient presented on Thursday, September 23, 2021 to ask the pharmacist about a "reaction" he had 2 weeks after getting the Janssen vaccine. This vaccine was given on 5-5-21. The patient had a round, raised area about 3 inches in diameter on either side of his neck going down into the shoulder area. Each area looked like it was puffy and fluid filled; it wasn''t hard.

Write-up: Starting within minutes of receiving the vaccine, patient starting having dizziness and nausea. The next day, starting spiking fevers to 103 degrees, which one month later, have improved (now are running around 100-101) but not yet subsided. Also has had persistent headaches, chest tightness, nausea, and body aches daily through this time as well.
