Write-up: Rescue Ambulance Report: ON SCENE TO FACILITY FOR A 35 YEAR OLD M IN MILD DISTRESS, AOX3 GCS 15 W CC CHEST PRESSURE THAT BEGAN AT 4AM. PROVIDER IMPRESSION CHEST PAIN CARDIAC. PT STATES HE TOOK HIS FIRST DOSE OF JNJ VACCINATION YESTERDAY NOON. TODAY IN THE AM PT HAD DIFFICULTY SLEEPING AND WAS RESTLESS THROUGH THE NIGHT. PT WAS ABLE TO FALL ASLEEP AT 2AM AND AWOKE AT 4AM WITH CHEST PRESSURE, PALPITATIONS, AND FELT UNWELL. ON ARRIVAL OF RA PT HAS 5/10 CHEST PRESSURE WITH NO LASTING RELIEF FROM 3 SPRAYS OF NITRO. 162MG ASPIRIN ADMINISTERED, SINUS TACH ON 12 LEAD. NO LOC, NO SOB, NO NAUSEA OR VOMITING, NO NEURO DEFICITS, NO ABDOMINAL PAIN, NO OBVIOUS TRAUMA, NO OTHER COMPLAINTS. ALL MEDS AND TREATMENTS DONE BY ON SCENE PARAMEDIC . Diagnostic Lab Data: Urgent did a chest x-ray which came back good and also ran a blood test and checked oxygen level, both of which came back okay even though I was having issues swallowing during these tests.

Write-up: A few days after taking the Janssen vaccine, I stated to experience issues breathing and swallowing. Sometimes feels like I''m trying to breathe and swallow at the same time but nothing happens and I''m left gasping for air. I''ve had to carry a gallon of water around with me the last 3 week. Whenever I cannot breathe or swallow, I have to put water in my mouth quickly to get back to swallowing normal. I''ve been into the Urgent Care - Oxboro and a check x-ray was done along with a blood test. Nothing concerning was found according to the doctor and I was told to continue monitoring. It''s been almost a month and I''m still having lots of difficulties swallowing/breathing. Tonight, as I''m typing this out, is a lot worse. Have medical insurance which covers 3 visits annually and I''ve used those on rushing into the Urgent Care to get checked out - therefore, I haven''t been able to go into the ER to get checked out further since that''ll be an out of pocket cost.

The patient''s weight was 185 (unit unspecified), and height was 261.62 centimeters. The patient''s concurrent conditions included: alcohol use, nonsmoker, and pistachio and cashew allergy, and other pre-existing medical conditions included: The patient was young, healthy and very athletic. The patient was in amazing health before his stroke. The patient had no illness at the time of vaccination, no past history of similar event, no adverse event after any previous vaccination, no allergy to vaccine, or drug. The patient had no pre-existing acute illness 30 days prior to vaccination, no history of hospitalization in last 30 days, with cause. The patient had no Family history of any disease (relevant to vaccination) or allergy. The patient had no history of drug abuse. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, intravenous, and batch number: 1820095, expiry: unknown) dose was not reported, 1 total administered on left (exact location unspecified) on 07-JUL-2021 for prophylactic vaccination. It was reported that the vaccine was injected to a vein (incorrect route of drug administration). On 18-JUL-2021, (10 days after receiving the vaccine) the patient experienced severe headaches (migraines) at the back of his head, to the point that he could not work for the entire week. On the same day, the patient suffered from a tragic blood clot. On 24-JUL-2021, the patient visited general practitioner who prescribed a magnetic resonance imaging (MRI), pain reliever, and muscle relaxant. On 24-JUL-2021, the patient experienced a seizure at home and he was rushed to the emergency room (ER), and admitted to the hospital; where he suffered a massive stroke in his sinus which led to bleeding and the doctors diagnosed as catastrophic brain damage. On the same day, the patient also had cerebral venous sinus thrombosis, cerebral haemorrhage, anticoagulant therapy, computerised tomogram head abnormal, mechanical ventilation, mental impairment. On imaging, he was found to have extensive dural venous sinus thrombosis with hemorrhage. He was treated with anticoagulation, anti-seizure medication, invasive mechanical ventilation, and transferred to alternated hospital for consideration of endovascular treatment. On an unknown date, the patient had a craniotomy and surgery to remove the clot. It was reported that the patient was currently on life support. The doctors had tested his blood for any other blood disorders or potential culprits for a stroke, but none had been found. His platelet levels had led them to conclude that this was caused by the J&J vaccine (platelet count abnormal). Laboratory data included: Head computerised tomogram (CT), there was near occlusive dural venous sinus thrombosis in the superior sagittal sinus torcular and right transverse sinus as well as some right-sided cortical veins at the vertex. There was a sizeable parenchymal hemorrhage centered in the high right frontal lobe as above, compatible with hemorrhagic venous infarct in this context. There was no evidence of large vessel occlusion, high-grade stenosis, aneurysm involving the intracranial arteries. On 28-JUL-2021, Laboratory data included, MRI that confirmed patient had a hemorrhagic stroke. On 17-Aug-2021, the patient continued in the hospital and was kept alive via breathing and feeding tubes. The patient had exhibited some eye movement but was unable to communicate. This spontaneous report received from a health care professional via VAERS concerning a 33-year-old male of unspecified race who experienced Cerebral venous sinus thrombosis and Cerebral hemorrhage 16 days post vaccination. As of this report, Patient''s height , weight, and concomitant medications were not reported. The patient had no relevant past medical history . Sixteen days post vaccination the patient presented to the hospital with recurrent seizure and declining mental status after persistent headache for previous several days. On imaging, he was found to have extensive Dural venous sinus thrombosis with hemorrhage. No platelet count was provided. He was treated with unspecified anticoagulation, anti-seizure medication, invasive mechanical ventilation, and transferred to alternated hospital for consideration of endovascular treatment. Head computerized tomogram (CT) shows near occlusive Dural venous sinus thrombosis in the superior sagittal sinus torcular and right transverse sinus as well as some right-sided cortical veins at the vertex. There is a sizeable parenchymal hemorrhage centered in the high right frontal lobe ,compatible with hemorrhagic venous infarct. Information is limited in this case, and the occurrence of Cerebral venous sinus thrombosis and Cerebral hemorrhage could represent background incidence of such events in the general population. However, a relationship with Janssen Covid-19 vaccine cannot be ruled out and thus the relationship is considered indeterminate.

Write-up: Chief Complaint pt in ENDO to get MAB- sudden onset of not feeling well, diaphoretic and almost passed out per endo staff. Denies pain or any other symptoms. hx of chronic afib- per ENDO pt had a run of 16 PVC''s while on monitor History of Present Illness Very pleasant 69-year-old gentleman presents to the emergency room upon referral from outpatient infusion. He became ill with some body aches headache cough and hiccups starting on Monday. He got tested for and was diagnosed with Covid on Wednesday at orchard drug, and had been referred to get monoclonal antibody infusion. As he was preparing for that, he was being monitored on telemetry and was noted to have some nausea and lightheadedness that corresponded with 16 beats of a wide-complex tachycardia. He was mildly symptomatic with this, it self resolved. Because of this the process stopped and he was referred to the emergency room for further evaluation. I have been informed that Dr, has seen him and recommended admission for observation close monitoring. He does have a history of paroxysmal atrial fibrillation and follows with electrophysiology Dr.. With known recent diagnosis of Covid on Wednesday, administering Regeneron here today Not repeating test today

Write-up: After receiving the johnson johnson vaccine I began to feel a bit of chest palpitation on my drive back home. Early the next morning I felt extremely winded I checked my vitals. My B/P was significantly lower than normal and my heart was racing averaging 119 to 128. I felt dizziness and as if I''d faint if I bent down. Massive craking headache and chills ensued. I decided to go the urgent care where they did and ekg and found abnormal reading that doesn''t correlate with my normal ekg when I go to my provider. They insisted that I betaken to the ER via ambulance. The ER also reproduced the same abnormal readings. I was advised to see my primary. And my primary advised for me to see a cardiologist. Soon after my hearing in my left ear became diminished with ringing and a blood shot developed in my right eye.

Write-up: I have had low platelets, in the 50''s, after the J&J shot, it went down to 38/41, and recently now down to 25. For Cirrhosis of the liver, this is abnormal it should fall not below 40 and perhaps coincidental, but suspect the J&J shot contributed to the the further decline in what was otherwise a somewhat stable 50 or so platelets I had achieved for some time. The J&J I have read in journals and other media suggest possibility that it may have contributed to further decline of my platelets. I wen to the ER as the Navy doctor ordered me after I indicated I had been tasting blood in the back of my thought for day.

Write-up: 37yo M with 6 days of diffuse, constant chest pain after receiving J&J vaccine on 16JUN. Patient reporting SOB and increased symptoms when laying flat. Write-up: The next day after having the vaccine, I had some sores underneath my nose and I was always hot. It felt like I was having menopause again. Then I got sick a couple of days before 07/23/2021. It was really terrible. My daughter tested positive for Covid. I contacted my doctor and told them about it. They sent me to get a test at urgent care. The next morning I got notified that I was positive. I also had a home test 2 hours before getting the test at urgent care and that one was negative. I got quarantine through my birthday. It felt like I had 5 different flues all going at the same time. The sores got bigger. I got them on my lips. I had a terrible fever. I had headaches. My ears were clogged up. My nose was running. I was tired all the time. I feel like I still have some brain fog sometimes. I still get some fever sores but I am taking some medication every day. When I had diarrhea they also gave me something for that. They also gave me something for the dry cough. It has continued with the dry cough off and on. Write-up: HIGH BLOOD PRESSURE; COULD NOT SLEEP AT NIGHT; RASH; ITCHINESS ON ONE SPOT AROUND THE HEART; This spontaneous report received from a patient concerned a 69 year old female The patient''s height, and weight were not reported. The patient''s concurrent conditions included: high blood pressure, diabetes, no alcohol use, and non-smoker, and patient had no known allergies. Patient had no drug abuse or illicit drug use. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 1805025, and expiry: 23-AUG-2021) dose was not reported, 1 total, administered on 11-MAR-2021 for prophylactic vaccination on left arm. No concomitant medications were reported. On 13-MAR-2021, the patient experienced rash and itchiness on one spot around the heart which resolved in 3 weeks. Three to four weeks after vaccination on 01-APR-2021, the patient experienced high blood pressure and could not sleep at night. Patient''s daughter who was a nurse took her to the hospital. They tested her for brain bleed and stroke all negative. Patient also underwent Laboratory data included: Blood pressure (NR: not provided) high (250/150), Echocardiogram of the heart (NR: not provided) negative, and Eosinophils (NR: not provided) really elevated. They said that was because of the vaccine. They raised the dose of her blood pressure medication and added a second one and it took 2 weeks before her blood pressure came back to her normal. They released her the same day and sent her to a cardiologist which mentioned the same thing that it was because of the vaccine. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. The patient recovered from high blood pressure on 15-APR-2021, itchiness in one spot around the heart, and rash on 01-APR-2021, and could not sleep at night. Write-up: At approximately 2 PM which was four hours after vaccination patient began to become ill. He developed headache, fever, chills, nausea and vomiting. He continued to have persistent vomiting throughout the night. He was seen in ER. He reports that he was given 2, IV''s of Ondansetron (4 mg each) at which time it brought his non stop vomiting under control. He was also given a shot of Toradol for his headache. He was given a prescription of Ondansetron and told to take Tylenol or Advil for his headache. His symptoms required that he take these medications for 3 days. Patient was home sick for 24 hours after ER visit and then was able to go to work again in the ER using his medications.

Write-up: Patient was nervous about getting this vaccination specifically and additionally did not like needles. Husband reported that it was normal for patient to faint with vaccinations. Vaccine was administered and patient remained sitting in chair with husband and myself by her side. She stated that she was getting very warm so I provided her with some air (by opening door). Patiently fainted momentarily in chair and then recovered quickly. This happened twice with approximately 1-2 minutes in between fainting episodes. Patient was alert upon recovery. Patient fainted a third time (approximately 1-3 minutes after episode 2) however when she fainted a third time, the patient went rigid in the chair and seemed to seize a few times. Emergency services were contacted and arrived on scene. Patient was alert and had recovered. Patient did not remember the events that had occurred other than the fact that she told her husband and I that she was getting very warm. Patient was taken via ambulance to ensure stable status.

Write-up: CHOKED ON DRINKS; DIFFICULTY SWALLOWING, EVEN LIQUIDS; BOUTS OF SLURRED SPEECH; MUSCLE CRAMPS; MAKING IT DIFFICULT TO STAND, BEING OFF BALANCE; DIFFICULT TO WORK; WEAK MUSCLES IN NECK AND SPINE; This spontaneous report received from a patient concerned of an adult female. The patient''s height, and weight were not reported. The patient''s pre-existing medical conditions included: Patient was healthy before vaccination. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 1808982 expiry: UNKNOWN) dose was not reported, 1 total administered on 10-APR-2021 on left arm for prophylactic vaccination. No concomitant medications were reported. After receiving the vaccine, the patient experienced deteriorating health. On an unspecified date, the patient had muscle cramps, being off balance, made it difficult to stand, as well as weakness in the muscles in her neck and around the spine. She also noted that lately, the muscle cramping was worse and she had bouts of slurred speech. She had difficulty swallowing, even liquids, noting she choked on her drinks. It was reported that it made difficult for her to work. She had numerous healthcare professionals (HCPs), including rheumatologists and neurologists. On unspecified date she visited Emergency room and Physician office for events difficulty swallowing even liquids, weak muscles in neck and spine, bouts of slurred speech, muscle cramps and making it difficult to stand. On unspecified date she had numerous tests done, including C-reactive protein test (CRP) and it was normal and a spinal tap was negative for meningitis. She felt that she was declining rapidly. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. The patient had not recovered from difficulty swallowing, even liquids, weak muscles in neck and spine, bouts of slurred speech, muscle cramps, making it difficult to stand, being off balance, and difficult to work, and the outcome of choked on drinks was not reported. Write-up: 80 y.o. male with a PMH of COPD, CHF, HTN, DM II, hx of DVT, afib, pacemaker, depression, PAD, HLD, depression, and recent viral pneumonia who presented to ED with shortness of air, worsening productive cough, generalized weakness, loss of appetite, body aches, subjective fevers, and chills. Patient presented to ED and was found to be hypoxic with spo2 of 80% requiring supplemental oxygenation and eventually had oxygen increase to 40L 70%. Patient had one episode of hypotension that resolved with 500ml fluid bolus. MICU was consulted due to increased high flow settings and deferred patient to progressive care. On assessment, patient''s o2 saturation of 87% with RR of 35 on 35% 30 liters high flow nasal cannula. Increased the patient to 50% 35 liters with o2 saturation of 92%. Constitutional: Positive for appetite change, chills, diaphoresis, fatigue and fever. Negative for unexpected weight change. HENT: Negative for congestion, hearing loss, sore throat, tinnitus and trouble swallowing. Loss of taste/smell Eyes: Negative for pain, redness and visual disturbance. Respiratory: Positive for cough and shortness of breath. Negative for wheezing. Cardiovascular: Positive for palpitations. Negative for chest pain and leg swelling. Chronic palpitations. #Acute on chronic hypoxic respiratory failure 2/2 COVID 19-PNA c/b CAP in setting of severe COPD and HFrEF - reports of severe COPD on home O2 (unsure of settings) and HFrEF 20-25% - COVID-19 positive on 9/6/21; s/p J&J vaccine earlier this year - CT PE 9/6: negative for PE, b/l GGO and consolidation, L$gR. emphysematous changes w/ peripheral blebs and airtrapping. Some peripheral honeycombing concerning for fibrotic changes with traction bronchiectasis - elevated d-dimer, LDH, CRP, and procal - 9/6: started on remdesivir + dexamethasone 6 mg daily, CAP coverage with ceftriaxone and azithromycin - not currently a candidate for barcitinib per ID due AKI - on transfer to MICU, was on HFNC 60L @ 100%, but this is likely 2/2 agitation and patient intermittently removing cannula. Precedex started and able to wean HFNC to 50L @70%. Plan: - Continue ceftriaxone for 5 days (thru 9/10) and azithromycin for 3 days (thru 9/8) for CAP coverage Patient continued to decline after admission as expected ultimately to his death on 09/16/2021 at 1134AM . Patient was pronounced dead by hospital bedside RN.

Write-up: I gave Patient his Janssen COVID19 vaccination today at about 10:10 A.M. today Oct. 11th, 2021. I had him wait in front of pharmacy, after about 5 min he said he felt light-headed so I went out to him. I asked him if he could lay on the ground and put his feet up on the chair. He said yes but that instance he lost consciousness and started to seize. His seizure lasted about 30 seconds then he slowly returned to a conscious state. He was really sweaty and sleepy. He was yawning. He was able to say his name and what day it was. As soon as he had lost consciousness I had had Staff call 911. EMS arrived at about 10:30 A.M. and by then he was said he was feeling a lot better. EMS stated he was a bit tachycardic. Pt reported no diabetes, heart issues, h/o stroke or seizures and no allergies except for penicillin and bees. Said when he was 6-7 years old he had a condition of vaso-vega (fainting condition) which he was told he grew out of by around 7 years old.

Write-up: I started having steady chest pain in the center of my chest and into the breast muscles. I still have it but not as bad. I went to the ER and they did an echocardiogram and blood work in July. They thought it was angina so I went to have a stress test and nuclear imaging. They said everything looked normal. I did not do anything to strain muscles or cause chest pain. They did not prescribe any medications.

Write-up: I had been sick for like 3 days. I had a fever, I felt nauseous, and I had a headache that nothing would touch. On 07/10/2021 I passed out. I had to be taken to the emergency room by an ambulance because my blood pressure dropped so low. I wear glasses and I felt forward, and my glasses cut into my eye. I had to have my eyes stitched. They did blood work, EKG, x-rays, and CT scan. They came back normal. I went to the cardiologist, and he thinks it was just dehydration. He did a heart monitor, and everything was normal.

Write-up: Aside from arm soreness and flu-like symptoms which lasted one day, I had no adverse events after the vaccine. Two months after the vaccine, I noticed that my right eyelid was drooping. About six days later, I thought my right cheek near my eye looked flatter than normal. The doctor''s office told me to go to the emergency room, which I did. That was on 6/24/2021. Write-up: On September 7, 2021, I received my J &J Covid-19 shot in my left arm. Around September 9th, 2021, the inside of my left arm began to swell, was rosy red in color and was painful to the touch. The vein along the inside of my left arm was hard extending from the middle bicep to the break in my arm. Current Illness: Patient was admitted from 9/7 to 9/16 patient presented after being found unresponsive at home by his 13 year old child. Per EMS report, patient had 02 sats in the 70''s with possible seizure-like activity on scene. Labs on admission revealed WBC 17, K+ 5.6, Cr 6.57, BUN 70, AST/ALT 166/55, and elevated CK 10,739. Patient presentation concerning for septic shock with hypotension requiring vasopressors and empirically started on Vancomycin and Zosyn. Full infectious workup performed on admission with respiratory culture returned positive for MRSA pneumonia. The patient was treated with 5 days of vanc and Zosyn. But left on 9/16 despite oxygen requirements and MICU team explaining that he still required medical attention.

Write-up: Pt is a 52 y.o. male with a PMHx of pancreatitis, alcohol abuse, DM2, and OSA who presented to the ED on 9/19 for AMS. Patient''s family says he has been increasingly unresponsive over last 3 days. Today, he was largely unresponsive, they were unable to wake him up, so they called EMS. On arrival, EMS initial blood pressure 77/46 and he was hypoxic, so they needed to ventilate with bag-valve mask. They gave 2 mg Narcan, to which he was minimally responsive, then brought to ED for further evaluation. Upon arrival patient was found to acidotic with a VBG: 7.17/62/55/23, Creatinine 5.52, Anion Gap 24, Lactate 3.2, WBC 15.52. He was intubated in the ER upon arrival and given cefepime, flagyl and vancomycin. BP was 64/40 requiring initiation of a levophed drip. He was also found to be COVID-19 positive on this admit. Write-up: On Friday August 27, 2021 developed a numb face, and slight left droop, ears roaring and sever headache, went to ED . Had a CTA, MRI, and negative for acute stroke and sent home. Followed with my PCP MD due to no relief of symptoms. Symptoms had increased with ears ringing and roaring at night, severe headaches with insomnia, pins and needles in my legs that were very painful, and anxiety. He prescribed Xanax to help with anxiety and sleep and also recommended to follow up with neurologist for neuro symptoms. Symptoms with sleep and anxiety were slight better with Xanax and Tylenol. By the time I saw MD , face numbness had slight improved and facial drooping resolved, pins and needles in my legs continued at night, sever headaches, double vision, difficulty swallowing and very loud roaring in my left ear. He ordered an EMG and carotid ultrasound. My symptoms have seem to stabilized at a consistent state. I have pins in needles in my legs that is relived with gabapentin at night, but most continued. I have cancelled my vacation to the grand canyon that was pain for and missed 3 weeks of work.

Write-up: August 3, 2021 I started experiencing symptoms of covid 19. I had a mild cough, but i thought it was just a cold. I was out of state at that time. Write-up: April 11, in the morning (8am) I woke up with a heavy feeling in my chest. Pain in my right arm, I also had flu like symptoms. My head hurt and I was coughing. My whole body was aches and pains. I was cold and hot, and I was in bed for a week. On the 14th, I went to the ER . They did a lot of tests ? ECG, (high), BP 160//100, a chest xray which was normal, a lot of blood tests and nothing came abnormal, troponin was .04. everything came back normal and the only thing that was abnormal was my blood pressure. The doctor said they didn?t find everything and said to follow up with primary doctor. I didn?t think anything of it at the time so I went back to my normal routine. I noticed that I was out of breath a lot, so I figured it was maybe old age. As I progressed more and more, every time I walk up a flight of stairs, I?m tired. This is where I got worried. I work out 5-6 times a week with pilates and hikes with my girlfriends with no issues. And now I?m no longer able to do that. That was my main concern. On 9/23/2021, I wasn?t feeling well so I reached out to my doctor, I called & texted him and he wanted to do a video chat. He asked me if I had a BP machine, so I took my BP and took it and showed it to him ? 161/102. He said you needed to go the ER but I didn?t want to go. The last time I went, there was nothing found. He said the BP scared him and I went to the ER on 9/23. On that day I went to the ER and its packed. They said it would be a long wait, but that was fine. They took my vitals and they said my BP was way too high. I sat at the ER for 3 hours. They finally called me to the back & checked an ECG and there were abnormality. They took my oxygen and said my heart wasn?t getting enough oxygen. They ran a stress test on my heart and took a lot of blood for tests. Some came back with some info, but the bottom line was they couldn?t tell me why my blood pressure was so high and why my heart wasn?t getting enough oxygen because it was strong. I talked to my doctor to see what else he could do. I was in the ER for about 2 days, they didn?t admit me but they held me there for 2 days. They were overwhelmed with patients. I followed up w my doctor on 9/29 . We did another virtual doctor visit and he ordered an inhaler for me called Albuterol 90. He told me to use the inhaler when I needed it the most. He also ordered me to follow up with a pulmonologist and I have an appointment with them on 11/11 at 1pm . I still can?t catch my breath, even when I?m talking. I?m trying to go back to working out and I can only do 1 hour a day because that?s all I can handle. A constant pressure in my chest, I can?t catch my breath. There has been times where if I gasp for breath too hard, I get a sharp pain in my chest and I get concerned. My breathing is starting to get harder and harder. I know from talking to other people who have had the same vaccine that they have the same problems.

Write-up: When they first gave me my shot, in my chest felt like my bronchial tubes were crackling and I panicked like I was not breathing right, I sat for 20 minutes and it dissipated. Write-up: I woke up with headache different from my migraines, I took my migraine meds it didn''t help. I was shaking , like uncontrollable shaking and the ER, They could not find anything. I went home slept headache didn''t go away I went to a different ER. The gave a migraine cocktail and 5 day of steroids and it helped. Then I was on vacation a week later I got up to go to the bathroom and I was bleeding, I went to ER again they found nothing. I was home for a few days I felt weird Told my husband to call 911 I was shaking uncontrollably , the ambulance took me to stroke center they could not find anything. Few more trips to ER for headaches and my resting heart rate was 150. I went back the next day to same ER and they admitted me because nothing had changed, they gave me IV steroids the whole time I was there They did another CT and MRI still could not find anything. Been seeing a neurologist, gastroenterologist. I have a constant headache never goes away. I''m taking amitriptyline, ubrelvy , sioricet.

Diagnostic Lab Data: The patient has had extensive serologic testing for underlying rheumatologic disease with no evidence of underlying lupus, rheumatoid arthritis, or vasculitis. Inflammatory markers were initially significantly elevated with ESR of 97 and CRP of 7.9, which improved following steroid taper. Infectious etiologies were evaluated with HIV serologies, QuantiFERON testing for tuberculosis, multiple blood cultures, lumbar puncture, and urinalysis without evidence of underlying infection identified. CBC was obtained without evidence of blood dyscrasia and serum protein electrophoresis obtained without evidence of multiple myeloma. Extensive imaging was obtained with MRI of the brain and cervical spine with and without contrast obtained. CT of the chest abdomen and pelvis with and without contrast was also obtained showing some adrenal nodules but no obvious infectious or neoplastic processes noted on these. Patient has been referred to ENT for evaluation for atypical presentation of giant cell arteritis which was felt to be unlikely. She has been referred to Gastroenterology and is preparing for upper and lower endoscopy to rule out any possible GI based malignancy. I have ordered 24 hour urine metanephrines given persistent symptoms to rule out pheochromacytoma.

Write-up: Approximately 1 week following vaccination the patient developed daily fever up to 103? F, diffuse joint pain and myalgia, severe headache, drenching night sweats, and fatigue. The symptoms persisted for 8 weeks prior to presentation to my office with prior workup at previous PCP. She ultimately developed erythematous nodular rash in addition to the above symptoms. I obtained a punch biopsy of this and it resulted with findings of neutrophilic dermatosis consistent with either palisaded neutrophilic dermatitis or possibly consistent with Sweet''s syndrome. The patient was started on prednisone for this and had initial improvement in rash but had subsequent relapse. At this time she has been referred to Rheumatology, Dermatology and Neurology. She has been initiated on dapsone for attempted steroid sparing regimen for treatment of her rash however she continues to have daily fever, headache and night sweats.

Write-up: The first time I got an initial attack was about May 2, 2021. I felt a flushing sensation, my heart was racing, nauseous, dizzy, and my blood pressure was high at the time, but I didn''t know it. I had to lay down. I felt terrible for several hours. I just laid in bed, I didn''t know what was going on. Then I felt better the next day. I had a couple of these events off and on with same symptoms. I didn''t check my blood pressure so at the time I didn''t now it was high. May 10th I saw my doctor. I was also having pain in my gut. She did bloodwork and said to go to gastroenterologist. She increased the dosage of my blood pressure medication. I seen the gastroenterologist and she performed an upper GI and a colonoscopy but didn''t find anything. June 9th I went back to my doctor because I was continuing to have the same attacks. My blood pressure was 170/110. I have never had anything like this before previously before having the COVID vaccine. I got a really bad attack at work on June 17th. I work with senior citizens and help take care of them. I felt fine in the morning and then by 1:00pm when I was going to bathroom I was dizzy, vision was swimming, and I thought I was going to pass out. I looked at my heart rate and it was 130 BPM. I still didn''t know that my blood pressure was high at that time. I laid down, and I didn''t feel good at all. I tried to rest and stay calm. At that point I had to call my company and send someone to come in for me so they could release me. I felt my heart rate racing and fluttering. I went to urgent care and my blood pressure was 190/ 117. They kept me there for about 3 or 4 hours. By then my blood pressure came down, and my symptoms kind of subsided. They wanted me to go over to the hospital, but I didn''t want to. So did they did an EKG that showed my heart rate and blood work was okay. They released me, and then I went home. June 22nd I called my doctor. Before April 12 the last time I saw my doctor was December 2020 but since getting the vaccine I have been to the doctor so many times and taking so many different test. June 29th I talked to my doctor about my symptoms and she increased my blood pressure medication again. July 8th I had to leave work again. This time it was late at night. I had gotten a blood pressure monitor and it was getting high even while taking my blood pressure medication that was adjusted to a higher dosage. I went into ER again and they kept me overnight because of heart racing and blood pressure was so high. I asked them to check my adrenal glands. And I found out that I have an issue with my adrenal glands. They diagnosed me with hyperaldosteronism. My endocrinologist said that that diagnosis shouldn''t cause the symptoms that I have been having except for my potassium being low and having a racing heart. The doctors still don''t know why I am still having these attacks if my potassium isn''t low. I had to go to ER again in October 2021 because of an attack I had in September 28th at work. I had the same symptoms. My blood pressure was 180/110. My doctor boosted blood pressure medication again to an even higher dosage. I started a new medication for hyperaldosteronism which is spironolactone 50mg, 2xday. September 29 I got another attack and went to the emergency room. Since then I have still had some attacks. I have missed 10 working days from all of this over the course after getting vaccine. I have a couple more appointments coming up. Symptom causes are still unknown. I am going to see a more experienced endocrinologist next month. I have almost passed out twice. I have felt horrible ever since getting the vaccine. I really didn''t want to get the vaccine, but due to my previous health and my job I decided to take it. July 8th i was hospitalized. I have been to my doctor 6 times since April 2021. I usually see her every 3 or 4 months. Now I am in and out of the hospital.

Write-up: The patient was sitting on the medical exam chair and couple minutes after receiving the shot he told his mom that he feels light headed and moment after he blacked out and fell from the chair to the floor. He was out for couple seconds then opened his eyes but not responding much. We provided Epipen since we noticed he was kind of shaking. We called 911 and EMT showed up at our lab and took his vitals. His blood sugar was at 90, and blood pressure at 100/80. They recommended taking him to the ER to be evaluated for any possible head or neck injury from the fall. The mother said he had blacked out before at sight of blood.

Write-up: Patient received full contents of one vaccine vial (5 doses). Patient was sent to ED for evaluation but was asymptomatic. Patient reported fever (Tmax of 102) and low blood pressure overnight on day of event. Has been followed by Primary Care and Hematology since event. On 9/24, patient presented with malaise, mild back pain, headache, and bruise on hip. BP and vitals were normal. Labs, including CBC and coagulation panel, have remained normal throughout. Lung examination on 10/4 was normal. On 10/11, patient reported some numbness and drooping on right side for 5-6 minutes (no slurred speech or numbness down arm). Patient planned to be discharged from Hematology service after 10/18 appointment, if all remains the same.

Write-up: Severe headache started on 4/9/2021, around 10 am. Didn?t respond very well to medication. Took nurtec & Imitrex, plus Advil, Tylenol, ice, not affective. Headache intensity Continued thru weekend. Called dr on Monday, started prednisone pack, headache continued, on wed (4/14) was advised by dr to go to ED for cat scan to check for blood clots. IV meds helped headache temporarily. CT scan was normal. 4/16/21 saw Doctor, severe headache again. Worst headaches of my life. Increased prednisone. MRI 4/19/21. Started PT to help with cranial pressure. Headaches almost daily since that don?t respond very well to medication.

Write-up: I was fine on Friday after the injection. On Saturday morning I felt like a had the flu. On Sunday, I had a really bad headache at the top of my head that lasted until Thursday. Friday, the headache started to ease up but didn''t go away. On Saturday, I started getting stabbing pains over my left ear that felt like an ice pick. I started having hearing loss in my left ear and pain in my left cheek and eye. I went to the ER where they did bloodwork and a CT scan. The doctor didn''t see anything unusual and released me and told to schedule a follow up with my primary physician. I was unable to schedule a face-to-face appointment with my doctor, so I had a telehealth visit with her. By then, I was still having numbness in my face, intermittent hearing loss, and muscle pain in the back of my legs. She referred me to a neurologist and a ENT specialist, who hasn''t been able to see me yet. Dr. performed a CTV which came back clear, but I still had the numbness in my face. He said the vaccine caused inflammation of my brain that was causing the face numbness and scheduled me for a MRI on October 20. The pain in the back of my legs is so severe that I can barely walk.

Write-up: About 5 hours after receiving the vaccine, I began to exhibit flu-like symptoms (chills, aching/weak muscles, headache). After about 30 minutes of these symptoms, my body began shaking and sweating and I had a fever of 102. About an hour into the reaction, my muscles were constricting, body was shivering, I could not speak, and I was having major shortness of breath. By the time I was flown to hospital (about 2 hours after the reaction began) my fever registered at 104 degrees and the same symptoms previously listed were ongoing. At the hospital I was treated with epinephrine and symptoms began to settle after vomitting.

Write-up: Visual check of ear last Saturday, no pain, no qtip, It was fresh blood. going to a fund raiser and there was blood. Random bleeding of the ear. drops. Left ear. Normal healthcare was not open. went to ER. It was still bleeding, Caused not known to doctor. The membrane was intact and no infection. There was still liquid which throws the balance off and blood is very heavy and throwing my balance off. Got antibiotics and had no more problem.
