Write-up: Crohn''s disease; other things; Initial information received on 28-Jun-2019 regarding an unsolicited valid serious case received from a nurse. This case involves a male patient of unknown age who experienced crohn''s disease and other things, while he received vaccine MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE [MENACTRA]. The patient''s past medical history, medical treatment(s), concomitant therapy, vaccination(s) and family history were not provided. In 2010, the patient received a first dose of suspect MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE lot number not reported via unknown route and was going to get second vaccination. On an unknown date, The patient developed a serious crohn''s disease and non serious other things (unevaluable event) following the administration of MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE. This event (crohn''s disease) was assessed as medically significant. Nurse asked about any concerns with administering Menactra to a patient receiving Remicade and methotrexate. Nurse stated that she did not have the patient''s chart available, so she did not know whether the patient already had been diagnosed with Crohn''s in 2010 and already was receiving Remicade and methotrexate, prior to his first Menactra immunization, or whether the patient was diagnosed with Crohn''s and began treatment after having received the Menactra vaccine in 2010. No lab test was reported. Final diagnosis was other things and crohn''s disease. The patient was treated with INFLIXIMAB (REMICADE) for Crohn''s disease and METHOTREXATE (METHOTREXATE) for Crohn''s disease. The event outcome was not reported. Information on the batch number was requested; Sender''s Comments: Patient presented with Crohn''s disease after vaccination. Time to onset is unknown. Patient was treated with INFLIXIMAB (REMICADE) for and METHOTREXATE (METHOTREXATE) for Crohn''s disease. Further information including patient''s medical history, concomitant medications, laboratory investigations excluding other etiology would be needed to fully assess this case. Based on available information the role of vaccine cannot be assessed.

Write-up: has crohn''s; falling asleep/ came home and slept/walking coma for three weeks; very letharhic; barely could function in school; This case was reported by a consumer via interactive digital media and described the occurrence of crohn''s disease in a child male patient who received Men B NVS (Meningococcal B vaccine) for prophylaxis. On an unknown date, the patient received Meningococcal B vaccine. On an unknown date, less than a day after receiving Meningococcal B vaccine, the patient experienced crohn''s disease (serious criteria GSK medically significant), sleepiness, lethargy and activities of daily living impaired. On an unknown date, the outcome of the crohn''s disease was unknown and the outcome of the sleepiness, lethargy and activities of daily living impaired were recovered/resolved. It was unknown if the reporter considered the crohn''s disease, sleepiness, lethargy and activities of daily living impaired to be related to Meningococcal B vaccine. Additional information was provided as follows: The age at vaccination was not reported. The day of patietnt''s doctor visit, the patient was talking, walking, active mentally and physically. The patient was in a near walking coma for three weeks after vaccination. After the shot, by the time they hit the car, the patient was almost falling asleep, very lethargic, he was like that for three weeks, barely could function in school, came home and slept. But of course doctor said it could not be the vaccine and at the time of reporting, the patient had crohn''s, an autoimmune disease. The reporter stated the vaccines were not what they were cracked up to be.

Write-up: Crohn''s disease; is immunocompromised; there was no more data than that in the PI. Limited data due to caller hanging up.; received PNEUMOVAX 23 on 04/2015 and 05/2015; This spontaneous report as received from a patient''s mother, refers to a female patient of unknown age. No information regarding the patient''s pertinent medical history, concomitant medications, drug reactions and allergies was provided. On an unknown date in April 2015, the patient was vaccinated with a dose of pneumococcal vaccine, polyvalent (23-valent) (PNEUMOVAX23) injection (dose, route of administration, lot number and expiration date were not reported); on an unknown date in May 2015, she received the next dose of pneumococcal vaccine, polyvalent (23-valent) (PNEUMOVAX23) injection (dose, route of administration, lot number and expiration date were not reported), both as prophylaxis. On unknown dates (it was not stated if it was before or after pneumococcal vaccine, polyvalent (23-valent) (PNEUMOVAX23) vaccination), the patient experienced Crohn''s disease and immunodeficiency (as it was stated that she was immunocompromised). She was taking a chemo type pill every day for Crohn''s disease. The patient''s physician wanted to do the re-vaccination. However, the patient''s mother wanted to know if that was safe and was upset due to limited data according pneumococcal vaccine, polyvalent (23-valent) (PNEUMOVAX23). The outcome of Crohn''s disease and immunodeficiency was determined to be not recovered. The relatedness between the events and pneumococcal vaccine, polyvalent (23-valent) (PNEUMOVAX23) was not provided. Upon internal review, the events of Crohn''s disease and immunodeficiency were determined to be medically significant.

The patient''s mother, who was a registered nurse, believed that quadrivalent human papillomavirus (types 6,11,16,18) recomb. vaccine (GARDASIL) injections caused the patient''s Crohn''s disease but the prescriber did not feel it was related. It was unknown if patient received treatment for the adverse experience, if any laboratory study was performed or if medical attention was sought. At reporting time, patient was not recovered from Crohn''s disease. The causality assessment was not reported. Upon internal review, Crohn''s disease was considered a medically significant event.
