Write-up: This patient was extremely irritable the night of his shots and never seemed the same afterwards. He became withdrawn, lost eye contact, regressed in many ways including language and gross motor movement. He did not laugh as much and generally was sick. He would cry and have when he screamed for an hour straight. He lost his spirit. Diagnosed with encephalopathy.

Write-up: On 7/20/00, the day after receiving vaccines, pt was very irritable and sleepy. She slept nearly all day and then again at night. Her left leg and left arms were sore and red at the injection sites. On 7/21/00, 2 days, post vax, she developed a fever of 102.3F with it''s highest at 103.1F. She began to cry for prolonged periods up to 8 hours. She also had intermittent screaming for no apparent reason. Over the next week, she continued to be very irritable and did not want to walk or use the stairs which she previously had done. She wanted to be very still and not moved around. She began to wake in the middle of the night several times, screaming and unable to go back to bed. On 7/29/00, her fever returned between 100F-102F (ax). She would still cry and scream and would arch her back and cry harder as if in a tantrum when anyone would try to comfort her. On 7/30/00, she developed a rash over her entire body. She was taken to the doctor and dx''d with a post vaccine varicella rash. She was put on steroids and Benadryl. On this same day, she stopped talking and her appetite continued to diminish. She stopped responding to loud noises or any sounds other than music from her favorite TV shows. She became glued to the TV set and did not want to do anything else. She seemed physically weak and when she did walk, it was an abnormal gait with 1 shoulder up and her head tilted to the side. She then started to put everything in her mouth, as if she were a baby again. She continued with the eczema, nasal congestion and other symptoms. On 9/23/00, she began to have a fever of 102 (ax), severe diarrhea 4-5 times a day, not eating well (drinking only) and began to pull at her left ear. She was rarely making eye contact. We thought her PE tubes may have been clogged and consulted her ENT. Two days later, still with diarrhea she had abdomen distention and she had open sores on her vaginal and rectal areas. She had brown drainage from her left ear. I insisted on taking her back to the doctor and he ordered stool cultures which were negative and assured me that everything was fine with her ears and that she was not dehydrated. I called again the next day, to the pediatrician''s office and was very upset and expressing my concern regarding a change in my daughter''s behavior and physical symptoms. The nurse assured me over the phone that the doctor would call me back if he felt it was necessary to see her. The doctor put her on Bactrim and suggested re-inserting the tubes since she was not responding to verbal stimuli, which she had done on 11/1/00. She awoke from anesthesia and was very upset, biting me and thrashing about. From 11/20/00 to 11/27/00, she started with a rash on her cheeks and a fever of 101F. The rash continued and the fever rose to 102.1F. She was lethargic and sleeping most of the time, off and on. She continued to have nasal congestion, no appetite and fever for the next few days. She began to have diarrhea again and was crying constantly for no apparent reason. She was dx''d with left otitis media and virus and placed on Suprax and fluids. On 12/14/00, she had same symptoms and was put on Ominicef and Floxin. She received Prevnar vaccine. One week later, she was unable to balance and was continuously falling. Her left ear had pus and was obstructed. The doctor ordered Garamycin drops and for the next month, several attempts were made to remove the obstruction to no avail. Her behavior continued to deteriorate and had a fit in the doctor''s office and it took me 45 minutes to get her out of his office. On 1/11/01, she developed influenza. On 1/17/01, she had another PE insertion. On 2/12/01, she had a respiratory infection. On 2/8/01, she was seen by a pediatric neurologist and dx''d with an encephalopathy. She has seen a geneticist who indicated that she has a mitochondrial disorder which was likely caused by the DTAP, especially the Pertussis. She has seen an allergist and he noted the she demonstrates several allergic type reactions. Doc 198237 states that patient has a permanent metabolic disorder which will require drugs (medication) for the rest of her life. She continues to have sensory problems, auditory processing, speech,language and behavior problems to frustration. She is allergic to numerous foods.But she has improved since the incident. She has started to gain weight and grow again. She is severely behind her development as a result of the event.

Write-up: 3 weeks after shot, a chickenpox came up on chest and again about 2 weeks later on her leg (if rash flared up badly). On 11/20/00, the night after shot, legs were sore, loss of appetite and continuous cries of pain. By that weekend, still continuous screams of pain and rash broke out and "Mennen''s" products starting burning her skin (she had used all their products from birth with no previous problems). When rash breaks out, her right eye seeps as if she has infection in it. Loss of appetite continued until oral steroid was given (Florida), still has problems with it when rash comes back. Since 11/20/00, sleep was affected and would only sleep for a hour at a time and continues to scream of pain (when being picked up or held at times). She had diarrhea from shot until last month (April). The rash will go into blister and skin will seep; glands swell. We''ve changed her formula for a month (Similac to Soy) with no change. Changed detergents from Tide to Dreft and there is no change. Rinse all clothing and bedding 3 times and no change. Watched for reaction to any food (did not eat "food" until age 5 months, 3 months after shot, not until oral steroids). No allergies. No treatments just lotions, ointments until last of ointment. Steroid creme (way too strong) and antihistamine. No soaps, limited bathing (every 3 days) just water and limited time. Fondak, limited time, Cetaphil soaps and lotion, every other day and no oils. Allergic reaction to treatment with cream and antibiotic (threw papers at me). All may work for a couple of days, then quit.

Write-up: Information has been received from a registered nurse concerning a 6 year old female patient who on 11/08/95 was vaccinated with one dose of Varicella virus vaccine live. There was no concomitant therapy. On 03/31/00, the patient presented to a physician''s office with varicella lesions all over her body. The patient''s experience was considered to be disabling by the reporter. No prescription drug treatment was required for the experience. Follow-up information from a registered nurse reported that all of the patient''s lesions were scabbed over and no other problems were noted. Another patient (WAES #00061355) had a similar experience.

Write-up: The first symptoms appeared on 2/23/98 which were fever, vomiting diarrhea, dehydration. These occurred immediately following; a loss of eye contact and interest in engagement with others, repetitive movements and speech, echoalic speech, insomnia, destructive behavior without remorse or understanding of what was done. FOLLOW UP information received on 08/02/02 states, "Autism-socially impaired, sensory integration dysfuntion, speech delay, self-regulatory dysfunction".

Write-up: I would like to register an adverse reaction to the vaccines my child has received. I cannot give you a form which states 1 vaccine and it''s reaction, due to my belief that my son''s autism was caused be the inclusion of thermisol preservative contained in most of his vaccines, MMR and DPT, due to their load on his immune system. Here is a list of his vaccines: 1st Hep-B on 12/30/95; DTP-HIB #1 on 3/8/96; OPV/IPV #1 on 4/4/96; #2 DTP-HIB on 5/6/96; #3 DTP-HIB on 5/6/96; #2 OPV/IPV on 6/3/96; #3 DTP/HIB and #3 OPV/IPV on 7/8/96; #3 Engerix-B/2129A2/IM/RL on 9/25/96; #1 MMRII/0473D/SC/LL and #4 HIB-Titer/Praxis/M270PA/IM/RL on 3/12/97; #4 Acel-Imune/444258/IM/LA on 6/13/97; and Fluzone/Pasteur-Merieux/7F81852/IM/RL on 11/25/97. Doc 204161, 60 Day follow up states that patient has autism spectrum disorder, immune disfunction, and many others related biological disorders.

Write-up: Initially very ill with high fever up to 103.4F (orally), chills with increased sinusitis and chest congestion. Seemed to recover in 5-6 days. Shortly thereafter, he started demonstrating "ticks" with hands, eye, mouth, arms and legs. He also lost ability to read sight words that he had done previously. He became hyper, had short attention and was unable to learn to read. Child was 4 years old when vaccines were given. On 8/17/01, report filed 3 1/2 years after child was given multiple vaccines of the routine childhood schedule at age of 4. Pt had fever and possible URI symptoms following that evening. After an unknown period, the pt developed neurologic symptoms and lost developmental milestones. No medical evaluation of the child since that time was provided with report, therefore I do not have a dx of the current status. The family reports autism. While vaccines have been suggested as a cause, studies have revealed the majority of cases of autism are not likely due to vaccines. The cause of the autism is currently unknown. 60 day follow up on 11/04/2003: "Vaccine recipient recovered from the adverse events."
