Write-up: Menstrual bleeding started about 2 hours later. Fever, chills, aches, joint pain, sharp headache, around that afternoon at 7PM. Fever and chills went away after 24 hours. Joint pain and stabbing headache remained for about 48. Currently still bleeding on day 5 and getting occasional sharp headache pains that last around 5 minutes and go away for hours. Joints still feel a lingering ?soreness? almost. Last night, day 4 the cramps that accompanied the bleeding intensified greatly and were very painful. Today I have much more ?period like? cramping still. Previously cramps were there but minimal and occasional.

Write-up: On 4/8/21 awoke to intense pain in his left foot about 12 hours after receiving his 1st dose Moderna vaccine batch #038B21A. Over the next few days swelling and pain increased in the left foot moving around from the big toe, to ankle, to arch. His foot was swollen and extremely painful and he was not able to walk or put weight on the foot. On 4/18 he went to the clinic. The doctor took fluid from the big toe joint to test uric acid levels. Elevated levels of uric acid confirmed his condition to be Gout and Indomethacin was prescribed for pain and swelling. He has never had Gout or any kind of arthritis, joint pain or foot injury prior to the Moderna vaccine. As of 4/13/21 he is still unable to walk or sleep well due to pain and swelling in the foot. He is not able to work or resume normal daily activities.

Write-up: Thursday 04/08/21 - 1:35 PM Went to Fairgrounds to get the Johnson & Johnson - Janssen COVID-19 vaccine shot with my husband. Both of us wore our masks, walked in together, went through the line together, we both got the shot in our left arms and then set together and waited the 15 minutes before we could leave. A little later at home we were working on some landscape lighting and I felt a bit foggy, light headed and kind of out of it. I was drilling into masonry and the first hole went fine but the second one for some reason just wasn?t going and I touched the drill bit with my fingertip on my left hand to see if it had smoothed out and I knew in my head not to do it and that it should be hot as heck but I didn?t feel anything at all so later that evening when I took a shower my finger was burning and I knew I had burnt it. That evening I begin to feel crippling cold, my fingernail and toenail beds had turned purple and I was shivering violently. I got under a pile of blankets with my dog and tried to slow my breathing I could feel my heart beating in my chest and it felt like the air I was breathing was cold and frosty. I had a 104+ fever that night. My husband got me some aspirin a cold washcloth for my head a popsicle and water to help bring down the fever. And I was able to sleep on and off that night. When I woke up I had a 103+ fever and a tingling and numbness in my arms. Before the fever my arms felt tight and cold and I had a tingle that ran from my elbows to my fingertips. Again it was aspirin cold washcloth and lots of water to drink to try and get my fever down. Friday 04/09/21 Was spent in bed trying to drink water and sleep through it. My arm was sore, not at the injection site but the whole top part of my arm felt like I had been hit by a truck. I had a fever again that evening. Saturday 04/10/21 I thankfully woke up fever free but my arms were tingling. I had to take my husband to the airport that morning at 6:30 AM. I came back home and went back to bed until about nine when I woke up my arms were cold and sore and tingling it seemed to be getting worse in my left arm and I felt shots of electricity from my elbow down to my fingertips. At one point I took a picture because my left arm hurt so much more than the right and my fingertip beds were purple on my left and pink on my right. I took aspirin and took a nap. I couldn?t hold my phone with both hands hands like normal sitting in bed because my left hand goes cold and achy. Woke up worried that both my arms were aching and tingling from my elbows to my fingertips seems to be where it stays the most. Then that pain subsided and my lip started to tingle. After a few hours that stopped but the tingling in my arms is about constant not as strong sometimes, not in both arms sometimes, it seems to almost always be in my left arm and at one point scary enough that I almost went to the ER. Went to bed around 11:30 PM and the tips of the toes on my left side were tingling and it went to the whole leg. Sunday 04/12/21 I woke up with a coughing fit and mucus at around 1:00AM and my right leg with a little tingly at 4:24 AM awake again and coughing woke me had to get a drink and my stomach was buzzing and stopped now just back to the bottom half of my left leg. All of a sudden after a coughing and sneezing fit I got a tremendously sore burning throat out of no where that only lasted about five minutes. I noticed my left arm straining to hold up the pizza box tonight when my right arm had no trouble at all. Then a little later in the day stiffness in the back of my neck. It is a weird strong dry cough and I was scared to go to the ER because my symptoms were so varied and ever-changing I didn?t know what they would do with me and I didn?t want to just get stuck in there for days with him not knowing or making it worse. I told my husband ? It?s so random I?m a little scared no one is going to know what to do with me...?I?ve had issues with allergic reactions and no one being able to diagnose them and me having to figure it out on my own on the Internet that I had Thyroid problems and that I was allergic to the sun for a few years in the past and aloe vera... I didn?t wanna wind up going, them not fixing anything and having a huge bill to pay. But this whole time my body is buzzing and tingling and going through something strange. At this point I am just telling my family I?m okay and what random symptoms I have had since we spoke last. They are on standby to watch my children if I need to go to the ER. The constant buzzing, numbness, cold tingling in my arms is worrisome. Monday 04/12/21 I am use to the numbness and tingling being a constant thing on some part of my body... There are times when I feel fine but I know it won?t last long. We can add an upset stomach and diarrhea to the list. I had a metal taste in my mouth for a little bit today. Both arms legs and my lips have randomly buzzed, ached or tingled. The websites say call 911 for emergency or call your local medical provider for assistance so I called the local ER to see if they could tell me if my symptoms were normal or if it?s something I should come in for and they couldn?t give me any advice... I was told if you feel like you should come in then come in. I had a very strong sharp pain at my elbow of my right arm tonight that made me sit up in bed... Tuesday 4/13/21 My left arm is still sore if people touch me or if I try to sleep on my side like I normally do it hurts trying to hold my phone with both hands my arm still goes cold and achy. So far this morning my arms are both tingling and my left leg and my lips so at this point I just say I?m buzzy. 11:25 AM randomly my right arm decided to feel worse for a change but is back to a low buzz... after a few minutes. I find myself holding them up or lifting them when they hurt it seems to help a little to elevate my arms to my chest. I am just waiting to see what comes out in the media, or to see if this gets any better, or if they learn what to do about it... It isnt over, it hasnt stopped yet.

Write-up: Burning sensation with tingling and some minor numbness within 5 min, within 15 min started to feel dizzy. spoke with EMT on site and was told I could remain for an additional observation before returning home and that another 5 people had the same symptoms that morning and appeared to be fine after some time. After I arrived home, on or about 10: 45, I ate an egg sandwich and the dizzy sensation faded. However, severe fatigue set in and by 1 PM I went to lie down. On or about 1:30PM, I awoke with severe chills and a fever of 101.5. The Chills lasted from about until about 3AM on April 9th. The fever, however, went down after I took Tylenol at about 8PM. I remained in bed on April 9th until 8AM and proceeded to work from home with fatigue and a headache that would not go away with Advil. By April 10th the a majority of the initial symptoms abated and only a sore left arm with a slight tingling sensation remained. On Sunday, April 11th, my right ankle started to swell and left leg had some pain (behind the knee) . Elevated the leg and the swelling went down on the right ankle and left leg felt better after massage on the 12th.

Write-up: Vaccine administered at clinic. Pt is a 24yo male who received the J&J vaccine on 4/8/21. On Day 1 reported having mylagias, chills and pain at the site of injection. Write-up: Thought I should let you know I was vaccinated on April 8 with Janssen (J&J). I had a severe reaction - chest pain and back pain for the entire day. I alerted Drs., but I''m sure it should be reported. I took my ZyrtecD prior to vaccine, which I''m sure helped in some manner. I was better on Friday with lingering back pain. Write-up: Other than feeling warmer than usual within minutes after the injection at 0715 AM I had no other symptoms until 430 PM when I began to develop a headache, muscle aches and joint pain. It came on slowly but by 930 that evening I was shaking uncontrollably for about 3 hours with chills and severe joint pain. My headache grew worse and lingered for several days after as well. I still have a mild headache today (4/13/21) and feel lethargic and almost foggy.

Write-up: On the day after the vaccine, 4/8/21 I woke up and felt heaviness in both legs as I walked around my home.That lasted for 2 days. on Saturday 4/10/21 those symptoms diminished slightly and I felt sleepy all day long. On Sunday 4/11/21 I woke up with numbness and heaviness on the entire left half of my body and was unable to walk properly. That evening I went to the Emergency Department. After review of CT and MR, I was diagnosed with having a stroke. I remained there for about 18 hours . My symptions decreased to tingling , so Iwas discharged, no treatment received. As of today, my sympttions are reduced almost amost 50% and i am able to walk with a walker..

Write-up: In addition to the common side effects of muscular pain, strong headache, chills, instead of nausea, there were vomits, two times. After 4 to 5 hours of the vaccine shot in the left arm, the area was swollen, red and painful. The adverse event different than reorted one is that the day after, a bruise appears and still in pain.

Write-up: Nausea the day after vaccination, which went away after a day. Starting two days after vaccination, she started experiencing what she described as severe headaches. The severity of the headaches decreased but did not completely resolve over the week after vaccination. The injection site was sore and remained swollen and painful over a week afterwards. She also said that she started wheezing starting a week after vaccination.

Write-up: Severe headache/migraine and nausea 2 hours post vaccination. Took 1 dose of sumatripan (100mg) for what felt like a migraine approximately 4 hours post vaccination. Vomiting 7 hours post vaccination (do not ever have vomiting with migraines in previous history). Severe headache and nausea continued through the next day though no more vomiting. After consulting with doctors office approximately 20 hours post vaccination, took 1 dose of tylenol (500mg x2) to help with headache. Tylenol dulled headache but did not eliminate. Nausea continued throughout day. Approximately 48 hours post vaccination nausea ceased and headache was mostly gone.

Write-up: Very lethargic, muscle and joint aches , nausea. I had a dream I was beaten with a bat. That is how I was feeling at the time. Write-up: Situation Patient called the COVID-19 Nurse Holtine with post-dose vaccine related symptoms/guidance. Pt called PCP office after hearing several news regarding J & J Vaccine/side effects. Pt was advised to call Nurse Hotline. Noted from pt''s PCP notes from today 4/13/21: "has some anxiety at baseline." Background Patient received her J & J on 3/7/21 on the left arm. Number of days post vaccine (today''s date minus vaccine date): 6 Initial reaction was noted on : 1 day post vaccine: 3/8/21 around 8 or 9 am. Symptom Screening c/o Bilateral leg "achiness." Pt rated pain 5 out of 10 on the pain scale. Pt denies any hives, itchiness, wheezing, swollen lips, tongue, eyes, difficulty breathing. Chest tightness/pain. Denies any swelling, redness, or itchiness to the both injection sites and to BLE. Pt denies numbness and tingling on the bilateral toes. Pt denies bilateral calf pain. Pt was able to ambulate without any difficulties. Medication: Pt took one dose of OTC Aleve at 5pm at 3/8 with a good effect. Pt back to baseline until today. Pt denies any unusual changes. No distress noted. ALLERGIC Reaction Did potential immediate allergic reaction (less than 4 hours after receiving vaccine) or anaphylaxis to first dose of the COVID vaccine occur? No Within 24 hours of vaccine administration, did patient experience symptoms of a severe allergic reaction? No Assessment/Action Patient experienced a MOD reaction to the vaccine. Symptom relieved with over the counter medications. Patient answered NO to any of the Allergic Reaction questions. Write-up: Started with headache, moved quickly to temperature of 102.7, numbness in face, chills, severe chest pain, heart racing at 131bmp though I was laying down, difficulty breathing, nausea, difficulty swallowing, extreme fatigue, paralysis of arms, hands, legs, feet, inability to speak or move mouth to form words. All of these things occured within 30 minutes of starting, from 1:30-2:00pm. Ambulance was called for me as I was unable to breathe well or speak, losing consciousness.

Write-up: Patient with racing heart, trembling within 15 minutes. Blood pressure checked and result was 131/61. Patient complained of dry mouth and Benadryl administered. Patient continued to be monitored and within 5 minutes following Benadryl, heart continues to race, chest heaviness, trembling hands, tongue numbness, tongue swelling, slowness of speech and thought. Write-up: April 8 about 12 pm, swelling and extreme soreness at injection site. About 5 pm, both ears stopped up. About midnight, chills, muscle and joint aches and headache, Took ibuprofen morning of April 9. Relieved headache and upper arm soreness soon after, but stopped up ears continued. Sometime later, noticed ears were okay. Felt perfect for a few hours after that. About 1-2 pm noticed pulsating sound in right ear. April 10, pulsating and other sounds continued in right ear. Write-up: I woke up around 3am with my heart beating very quickly. It would not slow down. I put on my husband''s smart watch. My heart rate was 95-100 beats a minute laying in bed. When I went to the restroom my heart went up to 112 beats per minute. I took a baby aspirin, ate some chicken soup, and went back to bed around 6:30am. I slept soundly for 5 hours. My husband looked at my heart rate on his phone (connected to his smart watch) while I was sleeping. He says the rate was usually around 60 beats a minute while I was asleep. Write-up: Patient presented to local ED on 4/8 after having symptoms ~0400 with right sided numbness unable to move fingers on right hand. Patient also unable to use right leg properly to walk. Write-up: On Thursday, April 8, 2021, between 915 and 945 AM, I received the Janssen/Johnson & Johnson single dose COVID-19 Vaccine. As I have had adverse reactions to vaccines in the past, I was advised to wait at the vaccination site for 30 minutes after receiving the vaccine, and I did. Approximately 2 hours after receiving the vaccine in my left arm, I developed a red lacey-type rash all over my body. (This rash still persists today, Tuesday, April 13, 2021 at 3:37 PM and is relieved temporarily with consistent doses of Benadryl capsules, liquid or tablets.) On Thursday night (April 8), after receiving the vaccine, I began to have body aches and to feel fatigued around 7:30 PM. I took 50 mg Benadryl, 500 mg acetaminophen, 800mg ibuprofen and went to bed. I woke around 12:30 AM and was drenched in sweat and my clothes were wet with sweat. I urinated and went back to bed and was shivering uncontrollably from the time I got out of bed until I got back in the bed and bundled up?it was several minutes before the shivering stopped and I went back to sleep. At approximately 2 AM, I woke up to urinate again. I had a terrible headache, like my head was in a vice; my tongue, throat, and nasal passages were very swollen, and it was difficult to breathe through my nose at all. I took an additional 50 mg of Benadryl and a 500mg acetaminophen and went back to bed. On Friday morning I woke to a phone call from work checking on me. I had slept through all of my alarms and it was 8:35 AM. I was unable to report to work that day and went back to sleep and slept for most of the day, only getting up occasionally to get a drink and use the bathroom. That evening I was able to get up and do a few things with the children and run to the grocery store, however, I became very fatigued and went to bed early without eating dinner. On Saturday morning, when I woke up, I realized that I had slept for over 12 hours. I was groggy and fatigued all day and had a nap in the afternoon. That evening I was able to participate in family movie night and was feeling ok until about 10 PM. At that time, I began to feel very nauseated and had diarrhea, even though I had not eaten anything that would upset my stomach. I had intense abdominal cramping and felt like I would vomit if I moved around too much. I went to bed and had to get up to use the bathroom with diarrhea at least twice after the initial symptoms at about 10 PM. On Sunday morning, I woke up after sleeping approximately 10 hours and was able to get up without feeling too fatigued. I did chores and errands and began to feel fatigued in the afternoon and took a nap for about 2 hours. That evening I also had to rush to the bathroom with diarrhea, however, there was no nausea. I went to bed at about 10 PM. On Monday morning, when I woke up, I felt very groggy and did not feel like I had gotten enough sleep. By the evening on Monday, however, I had begun to feel a little less fatigued and more clear-headed. As of Tuesday, April 13, 2021, I have had the red lacey-looking rash intermittently since having the vaccine. I am already on several OTC allergy medications for seasonal allergies and continue to take those medications, however, I have increased my Benadryl intake from 25 mg in the morning to 50 mg in the mornings with 25 -50 mg every 6 hours or so, as well. The rash improves for a short time while I am taking the Benadryl, but it reappears within a few hours after taking the Benadryl.

Write-up: Around 6 pm the same day I received the J&J shot I started to feel chilly and a little nausea. I went for a walk for about 40 mins started to feel worse. I had body aches throughout my back and chest and in both arms, was freezing. Tried to eat dinner but couldn?t, too much pain and increased nausea. Tried to lay down, but began having a fast heartbeat and fast/some difficulty breathing. The pain in my chest was the worst pain I?ve ever felt. Continued feeling horrible with a fever of 100.7, nausea, headache pains throughout chest back and arms, freezing, and my arms and legs felt like they were becoming numb. Could not stand, very dizzy, tried to sleep through it. I threw up around 11 and did so a couple more times after that. Was finally able to sleep and woke up feeling much better. Arm is still sore at the injection point.

Write-up: 7 days after receiving J & J vaccine, patient developed a severe headache at approximately 1am. He had periods of time that he couldn''t recall. He experienced vertigo as well as nausea. He tried to text and it came out as a jumble of misspelled words and letters. He called me, his mother, and he was extremely hard to understand and couldn''t say a complete sentence without using words that made no sense. Him being in college I was 1.5 hours away. Write-up: Several hours after getting the vaccine, my back spasmed out and the pain was continuous and I still have the upper back pain today. That same evening I developed the chills, stomach pain. The next day I began experiencing a fever, headache, nausea, light headedness, dizziness, chills, back pain, lock jaw, some numbness on left side of face, zoning out or spacing out. My period also came two weeks early. I had some throat irritation as well as the feeling of being weak and some neck stiffness. All these symptoms seem to have gone away by Sunday evening, if not Monday morning. Write-up: In the middle of the night/early morning of 4/08 I began to experience symptoms of the stomach flu, with extreme nausea, some vomiting, and extreme diarrhea. I could not keep any food or water in. Symptoms started to subside on the evening of Friday 4/09, but came back full force on Saturday 4/10. Write-up: Patient received Janssen COVID 19 vaccine and within a few minutes became very lightheaded and dizzy. Patient sat down to try and relieve symptoms and after sitting a minute proceeded to feint. She regained consciousness within a few moments and was then disoriented for a short time. EMS arrived and evaluated her and since she was pregnant she agreed to go with them for further evaluation. We followed up with her the next day, she was feeling better having recovered fully and was told at hospital everything was fine medically with her health and her pregnancy.

Write-up: Initial symptoms was exhaustion and nausea around 12:00 pm. . Headache, chills and trouble breathing followed. Symptoms worsened as the day progressed. I triggered a high heart rate alarm on my watch while sitting and dozing about 6:00 pm. I ended up in bed at 6:30 that night and slept off and on with fever, chills, headache and throbbing arm pain until 7:00 am 4/9/2021. I had to use my inhaler during the night to help breathe but do not remember the time. The next day I was much better with the exception of tiredness, nausea and headache throughout most the day. I have pretty much fully recovered on Saturday 4/10/2021 with only tiredness continuing to bother me as of the this report. I am sleeping more than usual. I did not notify the clinic of my reaction as they were closed over the weekend.

Write-up: Patient reports she had "metallic taste" in her mouth for roughly 3 hours post vaccination. Patient reported that on 4/8, she woke at 0100 with "toe cramps", she got out of bed, walked around and took some Tylenol, this resolved. Then on 4/9 around 1400, she had what she described as "the beginning of a migraine" in which she had blurry/wavy vision. She has had migraines in the past, however they are very infrequent. She is not treated for migraines due to their infrequency. In the past, she would take a Tylenol at the first sign of blurry/wavy vision and this would prevent a full blown migraine. On 4/10 at 0530, patient woke from "charlie horse" in left calf. She got up, walked around again and took a Tylenol, this resolved. Later that same day, patient developed the blurry/wavy vision again, took a Tylenol again and this resolved. On 4/11, patient reported soreness in left calf from previous day''s cramp and used heating pad. Patient has had 2 more episodes of blurry/wavy vision yesterday 4/12 and today, 4/13, which both resolved with Tylenol. Patient has not yet spoked with PCP. Write-up: presents with progressively worsening dyspnea on exertion for the last few weeks, much worse over the last one day. Patient reports feeling fatigued, having cough, and feeling more short of breath for the last day, and has developed central chest pain which is pleuritic in the emergency department. She reports that the chest pain is 4/10 in severity and does not radiate. Her caregiver at bedside reports that the patient has had worsening dyspnea on exertion for the last 3 or so weeks. Of note, she becomes dyspneic with limited exertion, such as going up a flight of stairs, at baseline. She had a fever of 101.4 ?F at home earlier today, but denies chills or diaphoresis. He had 1 episode of nonbloody vomiting earlier today. Denies nausea at this time. She received her second dose of the Moderna COVID vaccine yesterday, 4 weeks after the first. They note that approximately 5 years ago, she had a prolonged admission for which she was transferred to OHSU and was on ECMO for a time before making a nearly full recovery. She has had the baseline dyspnea on exertion since that time. Review of Systems Constitutional: Positive for fever and malaise/fatigue. Negative for chills. HENT: Negative for sore throat. Relatively low attenuation pericardial effusion is present which is new relative to the comparison examination. Pericardial fluid measures up to 1.7 cm in thickness. No thoracic aortic aneurysm or dissection. No mediastinal mass or lymphadenopathy. Two small enhancing foci are present in the liver, larger 1.8 in length which are similar to the comparison examination. Interval cholecystectomy. Visualized upper abdominal contents are otherwise without significant abnormality. There has been partial resection of the left 5th rib. Osseous structures are otherwise intact. IMPRESSION: 1. No evidence of pulmonary embolism. 2. Pericardial effusion and small bilateral pleural effusions which are new relative to comparison examination. 3. Two small enhancing liver lesions which are not further characterized by this examination but are similar to comparison examination. These could potentially represent hemangiomas. Assessment / Plan a 49 y.o. female with history of hypertension, developmental delay, DVT x1 not currently on anticoagulation, GERD, and anxiety who is being admitted for acute hypoxic respiratory failure, felt likely secondary to fluid overload from cardiomyopathy from pericarditis with associated pericardial effusion. #Acute hypoxic respiratory failure #Elevated Pro BNP #Cardiomegaly #Dyspnea on exertion #Pleuritic chest pain #Elevated D-Dimer Acute. Hemodynamically stable. On room air at home, and requiring 3 L via NC here. Considerations include new onset CHF, PE, PNA, PTX, pericarditis, pericardial effusion. Presentation most concerning for new onset CHF/cardiomyopathy from pericarditis with associated pericardial effusion, as demonstrated on CT angiogram. She has significantly elevated CRP which further supports pericarditis, and onset of symptoms approximately 7 to 10 days after receiving her first COVID-19 vaccine dose 4 weeks ago, which may be associated. Also possible viral or idiopathic. Mildly elevated BNP and pleural effusions suggesting cardiomyopathy. Patient had a fever to 101.4 ?F earlier today, per caregiver at bedside, and has had shortness of breath and cough, and thus pulmonary infectious process strongly considered, though clinically I feel infection less likely. She has a chronic leukocytosis and WBC is 21 today. She received ceftriaxone and doxycycline for pneumonia today in the ED, and may consider continuing these tomorrow. No evidence to suggest tamponade on bedside POCUS. - Admit to PCU with tele - Echo in AM - Ibuprofen 800 mg 3 times daily and colchicine 0.6 mg daily for treatment of pericarditis - Diuresis: 40 mg IV Lasix given at 0037 - Respiratory support as needed, wean as tolerated - RTDP - Incentive spirometry - Consider continuing antibiotics pending clinical status in AM #Leukocytosis Acute. WBC 21.5 on admit, increased from 15.1 two and a half months ago, but that was at end of admission for cholecystitis. Potentially due to inflammation, possibly infectious process. Write-up: On the day after the injection, a severe stiffness in both legs occurred. It was a soreness and stiffness similar to the day after running a marathon race . The soreness and stiffness would not change if I was sitting, walking, standing or kneeling. It was constant discomfort and irritation. It was not painful and therefore I originally did not go to medical. There was no discomfort in my left arm at the injection site. The discomfort increased for the rest of the day. A headache started in mid afternoon and kept increasing throughout the rest of day. I have not had a headache in years. Around 7:00 PM on Thursday, I had to sleep. I was extremely tired. My body had chills and I had to wear a sweatshirt and full set of clothes to bed just to stay warm. That is very unusual. I am usually throwing off covers every night throughout the entire year. I did not eat dinner because there was no appetite. I slept in bed for at least 12 hours. Typical night sleep = 8 hours. The next morning was no issue and therefore I did not go seek medical help because there are supposed to be side effects to vaccines. Now, I am reporting it because headache and the stiff muscles were identified on the news. However, it was around 24-36 hours and not the 6 days on the news.

Write-up: Almost immediately, I had pain in my arm that shot into my shoulder when the vaccine was being injected into my arm. I did not mention it to the pharmacist since I figured it was normal. I sat for 15 minutes as instructed, but other than feeling sore, I was normal. So we left. After about another 15 mins, My head started for feel "drunk". I contemplated pulling over since I was driving home, but decided to get home asap. As time went by, my head felt more and more cloudy, fuzzy, heavy and as if I''d been drinking alcohol. That night, I couldn''t sleep. I even took hydrocodon with tylenol (the pain was that bad) and an ibprofin. It helped a little. I finally got to sleep at 3am, Just to wake at 7am for work. All day, I could not focus, my head was super fuzzy. I had a bad headache, I felt super sick. My stomach also hurt and I couldn''t lift my arm. I tried taking a nap half way through the day because I kept making mistakes at work. It did not help how I felt. Since then, my symptoms have eased but now I seem to have a bad case of acid reflux, which I didn''t used to get randomly. I have not been to my dr yet and I haven''t done anything else for it. I was informed that I should report my symptoms by a family friend.

Write-up: I had the shot at 6:15PM and waited 15 minutes. I felt fine, 10 minutes later I started feeling my throat swell. I had a hard time swallowing and I could feel it getting a little worse right away and then I took 2 small sips of water and got home abd was afraid to eat or drink in fear of choking. This lasted from onset to finish 40 minutes and then I could feel it go away. I was very tired and went to bed. No other symptoms that night 24hrs later 6:20PM had teeth chattering chills and felt feverish 97 temp went to bed wrapped up and 2 hrs later felt fine.

Write-up: Chills and fever were accompanied by sharp joint pains, severe throbbing and pulsating headaches (primarily on the upper right side) and migraines that developing into throbbing and pulsing in the entire body lasting for 10 hours; three fever cycles and the first lasted 10 hours before the fever broke in a sweat; I could not sleep the entirety of the first night and experienced hourly urination and intense dehydration. After first fever cycle I was alert for an hour before second cycle began; after third cycle symptoms changed to nausea and dizziness that prevented me from being mobile without assistance or upright for more than a few minutes, by Saturday evening symptoms went away and I could resume eating, drinking, and walking again

Write-up: Eye pressure, blurry vision. Severe muscle aches, joint & body pain. Uncontrollable shaking. Terrible cold chills. Low grade fever. Extremely foggy brain. Extreme weakness. Write-up: I did not feel anything except the injection area right after the Janssen shot. It was great. Within the past week and a half, I feel a little tired and light headache. Sometimes, I had trouble sleeping at night. But, starting end of the second week, the symptoms got worse. I am suffering from a sleep disorder. I felt exhausted, bigger and longer headaches, joint pain, muscle pain. My Blood pressure went up. I visited my family doctor today.

Write-up: Janssen COVID-19 Vaccine EUA About 6:00 I started to get body aches. Took ibprofin - this helped until about 8:30. Body aches came on again with significant chills. Started vomiting about 2:00 am and had fever of 103. Vomited about 3 times throughout the night and once more the next morning. Continued to feel nausea and body aches all day Friday. Threw up again on Saturday. Started to feel better Sunday. Much better Monday. Arm swollen and red where shot was.

Write-up: Patient reports receiving vaccine at 4:00pm 4/7/21. Around 12:30am on 4/8/21 she developed severe nausea, vomiting, diarrhea, lethargy, confusion, and syncopal episode. Went to ER at 4am and was given IV fluids, nausea meds, labs. Was told a liver enzyme was elevated. Sxs persisted. She was seen for f/u at urgent care on 4/12/21 c/o upper abdominal pain and cramping, persistent nausea and vomiting, diarrhea, lethargy, dry mouth, dizziness. Patient was referred from urgent care to nearby Emergency Room for IV fluids and further evaluation/treatment.
