Write-up: After 15 minutes of observation after receiving the Pfizer vaccine, the patient complained of shortness of breath, palpitations, and feeling warm. Vital signs were initiated at 4:45PM: BP 162/78, RR 22, SpO2 98%, HR 77, Temp 98.1 F. 3 8oz bottles of water was offered and consumed. Client almost fainted, was shaky, and complained of numbness of left side of face and lips. Patient was placed on the floor in supine position. Vitals were reassessed at 4:52PM BP 160/94, RR 20, SpO2 98%, HR 80. EMS was called at 4:51PM and arrived around 5:10pm. Vitals were reassessed at 5:03PM: BP 162/88, RR 21, SpO2 99%, HR 74. When patient left the facility, she was alert, verbally responsive and in stable condition. Patient left the facility at 5:15PM and brought to the nearby hospital.
