Diagnostic Lab Data: Dr ran test such as strep, and kidney disease test and the results came back negative and discussed with me it''s an allergic reaction to the shot. He still is having rashes come and go and I''m so frightened of any other side effects! So far he''s had a couple of Dr visits and feel that if he gets any signs of other we will have to rush him to hospitals and he doesn''t ever want a shot again!

Write-up: Got vaccination on Monday evening stomach pain, Tuesday calls from school with headache, dizziness, stomach pains, weak. Returned to school on Wednsday and felt light headache sick to stomach and dizzy and swelling then came home that Wednsday night a rash had developed and so he went to physician and she ran some test on him like strep. I told her he hasn''t been sick he''s been perfectly fine until this vaccine he got. Test were sent off and reported back to her on 2/3/2015 with the whole weekend of rashes and swelling all over his body. The Dr then confirmed it was the HPV vaccine and that he may have short to long term of this rash with other possible side effects!

Write-up: This spontaneous report as received from a pharmacist refers to a 16 year old male patient. the patient''s concurrent conditions, medical history and concomitant medications were none. The patient completed, the series with GARDASIL (date unspecified). On 19-MAR-2015, the patient was vaccinated with the first dose of GARDASIL 9 lot # L001312, expiration date: 24-FEB-2017, intramuscular (dose was not reported), while sitting in the pharmacy chair. Within a few minutes (about 1-2) after receiving the GARDASIL 9, the patient began walking and reported feeling dizzy. the patient passed out and fell from a standing position onto the floor, where he hit his head, bit his lip and had now marks on his nose. During this time, the patient''s muscles were rigid. The patient woke up after about 2-3 minutes and stated that he felt like he had a dream. The patient was then escorted to a local hospital. The patient sought medical attention. The patient''s present status was unspecified by the reporter. The outcome of the event was not reported, at the time of the report. The relatedness between the events and the vaccine was not provided. Additional information has been requested.

Write-up: Patient immediately slumped over and then fell off the table. Patient landed on his head and started experiencing tonic clonic movements. Patient urinated on himself. When patient stopped having the tonic clonic movements and came to, he was unaware of the events that had happened leading up to him passing out. He did not know how he got on the floor. 911 was called immediately. Paramedics arrived and patient was transported.
