Write-up: pt reports vaccine was put in lt side of heart & had surgery on genitals;MD witnessed this on 25JAN99;Annual follow-up received on 11/14/00 provides no additional data. Follow-up: 4/2/99 admitted t Hosp. by coroner. Just awarded disability. Vaccine injected into heart near breast. Hx of sz. Pt seems to think that his mother and doctors were conspiring together to kill him. He thinks that his mother took the insurance money from his injuries and spent it on herself. He said he almost died several times and that he only received $3,000.00 and that he needs to be compensated for his injuries. He said when the good Lord takes him that he wants this to stop, because parents conspire to kill their kids for insurance money and the children either come out retarded or die from being vaccinated. (1) I''m requesting for new information such as copies of reports. (2) And do I O a hospital bills.

Write-up: Petechiae and fatigue led to CBC subsequent evaluation revealed aplastic anemia. 6/28, 7/1, and 7/5 vesicle (blister), swelling vaccine site. 7/2 and 7/3 muscle pain, itching on body. 7/11 scab vaccine site. 7/1 and 7/5 redness. 7/16 scab fell off. 7/1 itching vaccine site. Per 60 ay follow up pt has not recovered. On treatment with anti ? globulin and cyclospaule with improvement in aplastic anemia. An annual follow up report received 01/13/2004 adds: Not recovered. We do not believe that the aplastic anemia is related to vaccine, but cannot exclude. Aplastic anemia has not resolved.

Write-up: Uveitis, following a small pox vaccination by 1985. I lost the sight of my right eye-after several surgeries. MD who treated my uveitis, told me that in the 40''s there was a small pox scare and of those who were vaccinated, many were subsequently diagnosed with uveitis. President should be made aware that this small pox vaccinations could do a lot of damage to people''s sight. Floaters, photophobia-eye drops to reduce intra-ocular pressure. Finally lost sight of ret. eye to uveitis secondary glucoma. *F/UP report on 3/31/03: PT lost signt in her right eye to uveitis - triggered by a smallpox vax*. I finally became blind in the one (right) eye.
