Write-up: Eye swelling; Initial information regarding this unsolicited valid serious case received from Health Authorities from consumer (reference number-GB-MHRA-EYC 00265349) was transmitted to Sanofi on 07-Nov-2021. This case involved a 39 years old female patient (162 cm and 80 kg) who developed eye swelling while receiving INFLUENZA VACCINE and DIPHTHERIA, TETANUS, AC PERTUSSIS AND IPV VACCINE (DTPA IPV VACCINE) The patient''s medical history, past medical treatment(s), vaccination(s) and family history were not provided. At the time of the event, the patient had ongoing Pregnancy with the due date for the baby would be 18-Apr-2022. On 03-Nov-2021, the patient received a dose of suspect INFLUENZA VACCINE produced by unknown manufacturer lot Unknown via parenteral route in unknown administration site for Prophylactic vaccination and DIPHTHERIA, TETANUS, AC PERTUSSIS AND IPV VACCINE (DTPA IPV VACCINE) produced by unknown manufacturer lot Unknown via parenteral route in unknown administration site for Prophylactic vaccination. On 04-Nov-2021 the patient developed a serious event eye swelling 1 day following the administration of INFLUENZA VACCINE and DTPA IPV VACCINE. This event was assessed as medically significant as per reporter. No relevant laboratory data was reported. It was not reported if the patient received a corrective treatment for the event (Eye swelling). At time of reporting, the outcome was Not Recovered for the event. Write-up: Adverse Reaction: The next day after receiving the vaccine, I woke up with terrible body aches and fatigue like a had the flu. Over the next week, on my right side, my scalp became hypersensitive, and numbness crept down my neck, shoulder and right arm. In the second and third weeks, my arm became weak and uncoordinated to the point where I could not hold things or do much with my right, dominant arm. I additionally felt weak, fatigued and had terrible restless legs. During the fourth week after the vaccine, I began to lose balance and could not stand without holding onto something. I also lost strength and coordination in my right leg. My gait was not normal, and I could only take small steps. I was so fatigued that I could hardly do anything for myself. My body was completely weak, and my right side was partially paralyzed. Treatment: After initially seeing a doctor, I was immediately sent to a neurologist, and I remain under care of a neurologist to this day. An MRI was performed on July 6, 2011 which revealed transverse myelitis. I was given a five-day 1000 mg treatment of IV steroids (methylprednisone). I elected to do this out-patient due to cost and family at home. I then did physical therapy over the next six months. Outcome: The steroid treatment did not improve symptoms but did stop further progression. With time and therapy, I have regained sensation that was lost. Strength has improved on my right side but remains weak, and my gait is not always normal. In 2016, I was diagnosed with multiple sclerosis.

Write-up: Adrenal crisis/acute adrenal insufficiency; Tingling and numbness of feet and toes; Tingling and numbness of feet and toes; This literature case from was retrieved on 20-Oct-2021 from Vaccine Adverse Event Reporting System (VAERS) (reference number: 1751724-1) with additional information received on the same day (being processed together) and concerned a 21-year-old, male patient. The patient''s concurrent conditions included euthyroid sick syndrome and underlying autoimmune Addison''s disease/undiagnosed autoimmune adrenalitis. The patient''s concomitant medications were not reported. The primary objective of this study was to recognize adrenal crisis (AC) in patients without known adrenal insufficiency after vaccinations. On an unknown date, in preparation for school, the patient was vaccinated with influenza virus vaccine polyvalent (brand not specified; dose, route of administration and anatomical location: not reported) for prophylaxis. The batch number was not reported. On the same day, the patient was vaccinated with non-company co-suspect DTaP (diphtheria vaccine toxoid, pertussis vaccine acellular, tetanus vaccine toxoid; dose, route of administration and anatomical location: not reported) for prophylaxis. The batch number was not reported. On an unknown date, two days after receiving with influenza virus vaccine polyvalent and co-suspect DTaP, the patient experienced generalized body aches, fatigue and malaise. On an unspecified date, five days after vaccinations, the patient experienced mid-abdominal discomfort associated with scant vomiting with associated decreased appetite and fluid intake. On an unspecified date, six days after vaccinations, the patient experienced tingling and numbness of feet and toes. On an unspecified date, one week after receiving influenza virus vaccine polyvalent and non-company co-suspect DTaP, the patient presented to the emergency room (ER). His family had brought him to the hospital as he had been lethargic and appeared cyanotic. The patient denied fever, chills, rash, joint swelling, joint pain, neck stiffness or pain, sore throat, sinus drainage or pressure, urinary complaints, or bowel changes. He also denied recent travel or sick contacts. In the ER, the patient was lethargic, bradycardic with a heart rate of 30 beats per minute and had low blood pressure at 90/60 mmHg. On examination, he had a cold, as well as clammy skin that was mottled in color. Initial laboratory tests were: haemoglobin was 16.9 g/dL (normal range 13.7-17.5), sodium was 102 mmol/L (normal range 136-145), potassium was 6.2 mmol/L (normal range 3.5-5.1), aspartate aminotransferase was (AST) 299 U/l (normal range 15-37) and alanine aminotransferase was (ALT) 124 U/l (normal range 30-65). Additionally, the patient''s urine analysis was unremarkable. He was transferred to the intensive care unit (ICU) and started on fluids and given a prophylactic dose of ceftriaxone. A cortisol level was drawn, and the patient was given a dose of dexamethasone for the possibility of adrenal crisis (AC) in light of concurrent hyponatremia, hyperkalaemia, hypotension, and hypothermia. The patient''s blood pressure, heart rate, and coloration improved. Due to this positive response, he was started on intravenous (IV) hydrocortisone. Subsequent laboratory tests were: cortisol was 0.69 mcg/dL (normal range 4.3-22.4), adrenocorticotropic hormone (ACTH) was $g2000 pg/mL (normal range 7.2-63.3), dehydroepiandrosterone sulfate (DHEA) was <15 mcg/dL (normal range 80.0-560.0), total testosterone was 200 ng/dL (normal range 264-916), free testosterone was 0.8 pg/mL (normal range 9.3-26.5), 21-hydroxylase antibodies were 40 U/mL (normal range <1), thyroid-stimulating hormone (TSH) was 5.25 mcInU/mL (normal range 0.36-3.74) and thyroxine (T4) was 1.62 ng/dL (normal range 0.89-1.76). The low cortisol level prior to the treatment combined with improvement in clinical status following glucocorticoid treatment was consistent with adrenal crisis. Diagnostic laboratory testing that demonstrated an elevated ACTH along with a decreased dehydroepiandrosterone sulfate (DHEA), total testosterone, and free testosterone, and elevated 21-hydroxylase antibodies were consistent with autoimmune Addison''s disease. Thyroid-stimulating hormone (TSH) that was slightly elevated, with a normal T4, was suggestive of euthyroid sick syndrome. Computed tomography (CT) of abdomen showed no gross adrenal enlargement or abnormalities. Blood cultures did not grow any organisms. With continued treatment with steroids, the patient''s severe hyponatremia and other electrolyte abnormalities were corrected. On an unspecified date the patient was discharged from hospital. On discharge, the endocrinologist recommended starting the patient on maintenance mineralocorticoid. The patient was educated on the course of the disease and the need for exogenous steroids for maintaining good health. On an unspecified date, the patient recovered from the events of ''adrenal crisis'', ''tingling'' and ''numbness in feet''. In conclusion, acute adrenal insufficiency, also known as adrenal crisis, was a rare but known life-threatening condition. Patients with adrenal insufficiency may go undiagnosed until they have significant stressors inducing like in this case where adrenal crisis was induced by influenza and DTaP vaccination. All vaccines are known to cause fever, fatigue, and gastrointestinal disturbances by themselves, thereby making it difficult to diagnose AC. As infections are a major precipitant of AC, receiving future routine vaccinations should not be barred in patients with AI or those who have AC secondary to vaccinations. The reporter assessed the events as related to influenza virus vaccine polyvalent and DTaP. Adrenal crisis was induced by influenza and DTaP vaccination in a patient without a known history of adrenal insufficiency. It was also considered that as the patient had received two vaccines simultaneously, this potentially increased the stressor burden. Write-up: pregnant woman received Infanrix; pregnant woman received Infanrix / DOB :04-MAR-1992; received Infanrix instead of Boostrix; This prospective pregnancy case was reported by a other health professional via call center representative and described the occurrence of inappropriate age at vaccine administration in a 29-year-old female patient who received DTPa (Infanrix) (batch number 49TM3, expiry date 28th May 2022) for prophylaxis. Co-suspect products included dtpa vaccine pre-filled syringe device (Infanrix Pre-Filled Syringe Device) injection syringe for prophylaxis. On 12th October 2021, the patient received Infanrix and Infanrix Pre-Filled Syringe Device. The patient''s last menstrual period was on an unknown date and estimated date of delivery was on an unknown date. The patient received Infanrix at an unknown time during the pregnancy. On 12th October 2021, unknown after receiving Infanrix and Infanrix Pre-Filled Syringe Device, the patient experienced inappropriate age at vaccine administration and adult use of a child product. On an unknown date, the patient experienced vaccine exposure during pregnancy. On an unknown date, the outcome of the inappropriate age at vaccine administration, adult use of a child product and vaccine exposure during pregnancy were unknown. The pregnancy was ongoing. Write-up: The patient is otherwise healthy. Five days ago the patient received his MMR vaccine and DTaP. Four days ago the patient''s fever began. Three days ago the patient''s fever became higher to 100 to an a patient felt ill. He had a rash that began around the injection site. The rash was itchy. He also had a mild runny nose that was clear and requiring tissue. But he did not have a cough or a sore throat. Yesterday the patient was seen in his pediatricians office. He was negative for all of the following: COVID-19, influenza, and strep throat. His hematocrit, platelets and granulocytes were mildly high, Pediatrician was unsure of the diagnosis or the ideology. Last night the fever was 102 and patient complained of headache. Today, the patient is doing better and that he feels better. He is eating and drinking but not as much as he usually does. His runny nose has resolved. His fever tonight is lower, in the upper 99 range. However his rash persists as in the photographs and is itchy. Patient has had no diarrhea to suggest Coxsackie virus.

Preexisting Conditions: high blood pressure (well managed) scoliosis rhinitis and allergies to trees, grass, mold, dust mites, cats, dogs

Diagnostic Lab Data: Strep culture - negative 10/15 HSV 1 culture - pending 10/15 3 mm skin biopsy - 10/18; results received 10/20 and confirmed rash caused by viral eruption as suspected.
CDC Split Type:

Write-up: red blister like bumps developed on right index finger wed night post vaccine, then right middle and right pinky finger on thurs. Thurs night developed right tonsillar ulcer. Red spots on fingers were itchy and hands felt burning. OB asked me to follow up with PCP. PCP appointment friday, more spots appeared on hands. Continued to burn and itch. Negative strep test and HSV 1 culture ordered (result still pending). PCP stumped as I was not presenting typically for HSV infection, Hand Foot Mouth, or other but mentioned erthema multiforme as possibility. Had me follow up with dermatologist for monday appt. Saturday additional spots appeared on soles of feet, buttocks, and pubic area but not on mucous membranes. Sore throat and extreme fatigue all weekend. Treated with tylenol, benadryl, and topical cortizone cream for relief. Derm appt monday am. More spots developed by then, on shoulder, toes, hands. Derm suspected a viral eruption related to influenza vaccine. Took a 3mm core biopsy from spot on left wrist and prescribed clobetasol cream 0.05%. Today is wed... cream is making spots shrink, less red, no itching. Blisters now peeling. Still feeling fatigue, headache, and soreness. No new spots. At time of vaccine 10/12/21, was 29 weeks pregnant. As of 10/17, 30 weeks pregnant. Due Dec 26 2021. No prior history of adverse reactions to vaccines.
