This case involves a 1 years old male patient for whom reported temperature excursion of unknown cause at 0.7 Celsius for 25 hours and 24 minutes (Product storage error), while he received DIPHTHERIA/TETANUS/5 HYBRID AC PERTUSSIS/IPV(MRC5)/HIB(PRP/T) VACCINE [PENTACEL], ,MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE [MENACTRA], IPV (VERO) [IPOL], DIPHTHERIA-15/TETANUS/5 AC PERTUSSIS VACCINE [DAPTACEL] and DIPHTHERIA-2/TETANUS/5 AC PERTUSSIS VACCINE [ADACEL] [lot number and expiry date not reported] via an unknown route at an unknown administration site for prophylactic vaccination and INFLUENZA QUADRIVAL A-B MULTIDOSE VACCINE PRESERVED [FLUZONE] [lot UJ477AA and expiry date: 30-Jun-2021] via an intramuscular route at an unknown administration site for prophylactic vaccination on 20-JAN-2021. Medical history, medical treatment, vaccination, concomitant medication and family history were not provided. It was a case of actual medication error due to product storage error temperature too low. [Latency: at the same time of vaccination]. Details of laboratory data not reported. It was reported that, caller reported opened Ipol Multi dose vials and opened Fluzone quadrivalent MDVs. Caller reporting excursion occurring in Dece-2020. Write-up: This child died unexpectedly the following morning in her sleep. She has been diagnosed with SUDC (sudden unexplained death of a child). Preliminary autopsy did not uncover any medical issues or problems. There is further testing being done and it may be a few months before all tests are done and final report is available. I do not submit this event as evidence of the cause of this child''s death. However, I need to report this event as it is not clear whether or not the vaccine(s) are related. I had initially planned to wait until final autopsy results were available, but that is taking very long and I wanted to make this report. I submit it today and can provide final autopsy report when I receive it.

This case involves a female patient (also reported as a baby) who had a reaction as a child to the pertussis part of the DIPHTHERIA, TETANUS AND ACELLULAR PERTUSSIS VACCINE (DTAP) which resulted in convulsions, while she received DIPHTHERIA, TETANUS AND ACELLULAR PERTUSSIS VACCINE [DTAP]. Medical treatments, vaccinations and concomitant medication were not provided. At the time of the event, the patient had ongoing medical history of latex allergy (rubber sensitivity) which was hereditary and she was tested for this before childbirth. She has concerns about her son receiving as well. Write-up: Serum sickness-like reaction (SSLR); Swelling of her joints; A generalized urticarial rash; High fever; Worsening Arthralgia; Initial information was received on 27-Oct-2020 regarding an unsolicited valid serious case from other healthcare professional issued in a literature article. Literature abstract- Case Description: A 4-year-old female developed a generalized urticarial rash with swelling of her joints six days after receiving MMR, varicella, DTaP, and IPV vaccines. She was started on prednisolone by her pediatrician for presumed SSLR however was subsequently admitted with high fevers and worsening arthralgia. Her CRP, procalcitonin and ESR were all elevated and her respiratory viral panel was positive for adenovirus. Her complement levels(C3, C4) were normal. IV methylprednisolone was started with dramatic improvement and resolution in symptomatology over the course of 48 hours.She was diagnosed with SSLR due to routine vaccinations and was discharged on a prednisone taper with instructions to space out vaccinations in the future to elicit the causal vaccine. Discussion: SSLR''s are usually attributed to medications and self-resolve after discontinuation of the offending agent. SSLR due to MMR, varicella, DTaP, and IPV vaccinations have not yet been described in literature. Refractory or severe symptoms with high fever, debilitating arthralgia, and persistent urticaria can be treated with prednisone. This case involves a four years old female patient who experienced serum sickness-like reaction (SSLR) (Serum sickness-like reaction), generalized urticarial rash (urticaria), swelling of joints (joint swelling), high fever (pyrexia) and arthralgia (arthralgia), while she received vaccines POLIOMYELITIS VACCINE (INACTIVATED), DIPHTHERIA, TETANUS AND ACELLULAR PERTUSSIS VACCINE, VARICELLA ZOSTER VACCINE [VARICELLA VACCINE] and MEASLES VACCINE, MUMPS VACCINE, RUBELLA VACCINE [MMR]. Medical history, past medical treatments, past vaccinations, concomitant medication and family history were not provided. On an unknown date, patient was diagnosed with serious serum sickness-like reaction (SSLR)(unknown latency) following vaccination. The patient was hospitalized for these events on an unknown date. Write-up: Need my physician to provide the specific manufacturer/batch. Within hours of the vaccine, I felt irritable, feverish, tired for two days. I had the vaccine Friday the 14th. By Monday the 17th, I started bleeding, I passed a blood clot the size of a baseball. I went to the hospital and they believed I had a placental abruption. After monitoring me for two days, and the baby''s heart rate continuing to drop, I had to have an emergency c section 5 weeks early.

Write-up: Left thigh "lump" and redness reported by Mother 12/1/20 via phone call. Mother states that she believes this was where subcutaneous injection was given on 11/20/20. Mother noticed this on 11/30/20. Patient was seen by PCP on 12/1/20 and reported back to writer that the PCP feels this is a deep tissue bruise. Patient''s mother to monitor at home.

Write-up: she was a newborn and was breaking out in hives and rash all over her body and was screaming, not wanting to sleep. i thought she was a colicky baby but looking back on it she was getting allergic reaction to the vaccinations. but the doctors constantly told me that it was baby acne or eczema and it was until she was 9 months old and i got allergy test for her and shes allergic to eggs and peanuts because of these vaccinations. shes constipated, swelling in the eyes, always itching, hive and rash

Write-up: Patient''s arm was swollen and red at injection site immediately after vaccination. Redness and swelling continued for several days worsening to cover an area from shoulder to elbow, with limited range of motion in the arm since it hurt so badly to raise it. Patient expressed having a constant headache at her forehead for nearly a week. She was lethargic and cranky with fits of crying in between. (Highly unusual for her). She simultaneously (evening of vaccination) developed a rash on the backs of her thighs that became rough and eczema-like within a day. The areas (arm and legs) were hot to the touch, red, and her legs were cracking and inflamed. Consulted her pediatrician who said the reaction was in line with this vaccine. She suggested topical hydrocortisone cream (1%- otc) followed by Aquaphor 2xs daily for the legs. The eczema persisted for several months. The redness and scaly skin subsided in late August. Her legs sustained pigment loss where the reaction occurred. Her 4th dose of of DTaP on 9/29/2020 caused an immediate relapse, and more severe reaction including a low grade fever. We are currently treating the same location on backs of her legs for the resulting eczema. This time the area appears to be more tender and bleeds.

Aka vaccinated/Suspected vaccination failure; Whooped; This case was reported by a consumer via interactive digital media and described the occurrence of vaccination failure in a male patient who received DTPa (DTaP vaccine) for prophylaxis. On an unknown date, the patient received DTaP vaccine. On an unknown date, unknown after receiving DTaP vaccine, the patient experienced vaccination failure (serious criteria GSK medically significant) and whooping cough (serious criteria GSK medically significant). On an unknown date, the outcome of the vaccination failure and whooping cough were unknown. It was unknown if the reporter considered the vaccination failure and whooping cough to be related to DTaP vaccine. Additional details were provided as follows: The age at vaccination was not reported. This case was considered as suspected vaccination failure, since the details regarding completion of primary vaccination schedule, time to onset and laboratory test confirming whopping cough were not provided.
