Write-up: My son was born absolutely perfect, full term and healthy who weighted 7 pounds and had been meeting all his milestones prior to his 12 months'' shots. He had perfect eye contact and would socialize perfectly. After MMR and other vaccines at his 12 months, he became silent. After his 15 months shots, more adverse effects appeared. He had a short nap and became diarrhea and was constantly vomiting. He lost his eye contact and stopped responding to his name. He started walking on a tip toe and spinning and had been showing severe hand flapping. He would put everything into his mouth like his 8 months. He was staring at lights for several minutes and showed many repetitive behaviors like opening and closing doors for several minutes. He would eat only puree food. He had loved fruits prior to his 12 months but he stopped even touching them. He would hate new textures. Generally what happened after the vaccination he suddenly and immediately showed all the Autism symptoms.

Write-up: Immediately experienced arm pain. As the day went on, the pain escalated substantially. I essentially lost all use of my right arm the pain was so excruciating. My arm also swelled up and turned red and slightly purplish. (Took pictures of it, too.) I would try to move/lift my arm but absolutely could not. By late Sunday/early Monday (Jun 30/Jul1), the pain lessened some. The swelling went down by Wed., 7/3. It is nearly 2 weeks later, and I am still experiencing constant pain with sharp sudden pains when I raise my arm, try to sleep on my side, or use my muscles. I am concerned I may have permanent nerve damage now.

Write-up: Baby lost eye contact, lost focus, did not respond to parent when I spoke to patient, within 3 hours after both the DTaP and IPV shots at Dr. office on October 28, 2013. Eyes roll like seizures. I called and spoke to both the nurse and also Dr. on the phone about patient''s lost of eye contact and lost of focus. Baby had several vaccinations before his two shots of DTap and IPV vaccinations on October 28, 2013, when he was 8 months old. Baby lost eye contact, lost focus, and he did not respond when I spoke to him, within 3 hours after the shots on October 28, 2013 from Dr. office. The next day I called, and spoke on the phone with the nurse and Dr. about patient''s lost of eye contact and lost of focus, and Dr. told me that it is unusal. After his shots, his eyes rolled like seizures. Little patient also started flapping his hand, was speech delayed, and showed poor social communication skills. I have a video of his hands flapping. Actually, several videos of hands flapping. Dr. from Hospital Autism Center diagnosed little patient with Autism, level 2 moderate level AUTISM on June 19, 2015. Baby seemed very lethargic, non-alert, and tired after his August 6, 2013 Hepatitis B vaccine at the Hospital. Treatment: Pump breast milk until he was 5 years old, and fed him partial of my breast milk. Treatment: Speech Therapy, Occupational Therapy, Physical Therapy, ABA Applied Behavioral Analysis Therapy. Status: People in public sometimes tell me that he is abnormal when he talks, and they are wondering why he is not acting the same way as the typical kids, and I did not tell them patient has AUTISM. He is currently in an AUTISM program at a public school. Patient is a special needs child with an IEP Individual Educational Plan at school. Write-up: allergy; Facial edema; mouth hurt; crying; This case was reported by a other health professional via call center representative and described the occurrence of allergy in a 30-month-old female patient who received DTPa (Infanrix) (batch number 5553K, expiry date 1st February 2021) for prophylaxis. On 26th December 2018 10:51, the patient received the 4th dose of Infanrix (intramuscular) .5 ml. On 26th December 2018, less than a day after receiving Infanrix, the patient experienced allergy (serious criteria hospitalization), face edema (serious criteria hospitalization), oral pain (serious criteria hospitalization) and crying (serious criteria hospitalization). Infanrix was discontinued. On an unknown date, the outcome of the allergy, face edema, oral pain and crying were unknown. It was unknown if the reporter considered the allergy, face edema, oral pain and crying to be related to Infanrix. Additional information was provided as follows: The patient received Infanrix on the left thigh. Same day, the patient started with crying because the mouth hurt and patient had facial edema. The patient was seen at the emergency room on the same day at 11:52 pm and was admitted for 12 hours and then discharged the next day. The emergency room provided a diagnosis of allergy, unspecified. The patient''s mother also reported to the physician''s office that the patient would not be completing the vaccination series later on (5th dose or 2nd booster dose).

Write-up: On Saturday, by noon, patient had painful gas with grunting and vomited a little bit. By 8pm started with gas pain, grunting, weighing down, then became unresponsive. Ambulance took patient to ER, given rescue breathing, CPR, and shocks were required. At hospital patient was intubated, had arrhythmia, respiratory issues. Patient taken to air to Hospital were she is in ECMO now. Patient is still at the hospital now. They are treating her for myocarditis or cardiomyopathy. Patient has positive Adoneno virus screen test for some type of infection. Myocarditis is an adverse event to resuscitation.

Write-up: 27 hours after the vaccine, patient''s left thigh from where the injection was down to her knee only on the front side turned extremely swollen, red, warm, painful, sore, itchy with a rash. Her anus and vulva were also red.

Write-up: nurse accidentally injected Daptacel that expired on 16May2019 to a patient on 21-May-2019/no AE; Initial information received on 22-May-2019 regarding an unsolicited valid non-serious case received from a nurse. This case is linked to cases 2019SA143977 and 2019SA143962 (same reporter). This case involves a 5 years old male patient who was vaccinated with an expired dose of vaccine DIPHTHERIA-15/TETANUS/5 AC PERTUSSIS VACCINE [DAPTACEL]. The patient''s past medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included POLIO VACCINE INACT 3V (VERO) (IPOL POLIO VACCINE) for Immunisation; and MEASLES VACCINE LIVE (ENDERS-EDMONSTON), MUMPS VACCINE LIVE (JERYL LYNN), RUBELLA VACCINE LIVE (WISTAR RA 27/3), VARICELLA ZOSTER VACCINE LIVE (OKA/MERCK) (PROQUAD) for Immunisation. On 21-May-2019, the patient received a 0.5 mL dose of suspect DIPHTHERIA-15/TETANUS/5 AC PERTUSSIS VACCINE lot C5202AA, expiry date: 16-May-2019, via intramuscular route in the left thigh (reported as left leg). It was a case of actual medication error due to expired vaccine used. No adverse event was reported. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holders compliance with the requirements set out in Directive and Module VI of the Practices.
