Write-up: Woke up next am after vaccination with complaints of facial swelling. Went to ER and was released then had to return to ER again the next day and was once again released. Went to different ER and was hospitalized. Facial swelling worsened and began having difficulty breathing and as of today 01/11/2010 pt is intubated.

Write-up: Pt sat up from sleep and was unresponsive. Eyes rolled to the back of his head. Left side of face was "twitching," as were left upper and lower extremities. Seizure later generalized. 2mg Ativan was given IV by ambulance which did not break seizure. Seizure finally broke 45 minutes after onset after loading dose of Cerebyx given.

Write-up: Patient suddenly became tired and lethargic and refused to eat then she began vomiting over and over again. She dehydrated very quickly. Ran only a low grade fever. Complained constantly that her hands were tingly and her fingers were completely numb. She may Require IV fluids since she can not keep any liquids down. She has not been around anyone that was ill so I looked online and found that TONS of people have become ill from these vaccines, especially the live vaccine given in the nose which is what was given to patient. People are complaining of the exact same symptoms anywhere from one day to a month after receiving the H1N1 vaccine.

Write-up: 1/15/10 tingling, heaviness, and weakness hands and legs. Difficulty walking after 24 hours. Absent gag reflex on 1/17/10 so he was intubated and placed on vent. He was alert to give informed consent. Dr. at Hospital has dx him w/ symptoms consistent with Guillain-Barre syndrome. Pt is still hospitalized in ICU on vent.
