Co-suspect products included Hib (Hiberix) for prophylaxis, PNEUMOCOCCAL VACCINE (PREVNAR) for prophylaxis, DIPHTHERIA TOXOID + PERTUSSIS TOXOID + TETANUS TOXOID (DAPTACEL) for prophylaxis, POLIOMYELITIS VACCINE INACTIVATED (IPV VACCINE) for prophylaxis, DTPa (Infanrix) for prophylaxis and dtpa vaccine pre-filled syringe device (Infanrix Pre-Filled Syringe Device) injection syringe for prophylaxis. On 15th December 2021, the patient received the 1st dose of Shingrix (intramuscular) 50 �g, Hiberix, PREVNAR, DAPTACEL, IPV VACCINE, Infanrix and Infanrix Pre-Filled Syringe Device. On 15th December 2021, unknown after receiving Shingrix and not applicable after receiving Hiberix, Infanrix and Infanrix Pre-Filled Syringe Device, the patient experienced off label use, drug use for unapproved indication and inappropriate age at vaccine administration. On 24th December 2021, the patient experienced shingles and skin lesion. On an unknown date, the outcome of the shingles and skin lesion were not recovered/not resolved and the outcome of the off label use, drug use for unapproved indication and inappropriate age at vaccine administration were unknown. It was unknown if the reporter considered the shingles and skin lesion to be related to Shingrix, Hiberix, Infanrix and Infanrix Pre-Filled Syringe Device. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional details were provided as follows: The reporter was the patient and he is also a medical doctor. The patient received doses of Shingirx, DTaP, HIB, IPV, and Prevnar and experienced shingles. The patient was 48 years old but stated that he had a medical condition that warranted receiving Shingrix , which led to off label use, drug use for unapproved indication. On the same day the patient received DTaP, HIB and Infanrix , which led to inappropriate age at vaccine administration. The reporter stated that he thought the name of the DTaP vaccine he was administered was Daptacel. The reporter did not provide lot or expiration date of the vaccines administered. The reporter stated that on 24th December 2021, approximately 10 days after the vaccines, he developed shingles which was currently unresolved. he still had active lesions. The reporter also stated that if the patient was administered Hiberix or Infanrix (exact vaccines administered uncertain per patient) then that would be AE for inappropriate age. The reporter did not want to provide his email. The reporter consented to follow-up.

Write-up: My daughter had a seizure it lasted around 5 minutes. Her body became stiff and her hands were curled and her hands were shaking. After a few minutes, she opened her eyes but was unresponsive. She kept sleeping and around 2 am she yelled in her sleep and started to shake briefly again. She fell back asleep and I scheduled an appointment with her provider.

Write-up: vaccine was given and child started to itch leg within 5 minutes of giving vaccine. We looked at the site and multiple hives were noticed. 2.5ml/6.5mg of Benadryl were given. Itching and hives started to diminish within 10 minutes. Gave instructions for severe reaction at home and mild instructions.

This case involves a 20 years old female patient who had an anaphylactic reaction to the dtap shot because it had traces of dairy in it while receiving vaccine DIPHTHERIA, TETANUS AND ACELLULAR PERTUSSIS VACCINE. The patient''s past medical history, medical treatment(s), and family history were not provided. The patient had Anaphylactic reaction to previous vaccines. Concomitant medications included Tozinameran (Comirnaty) for Immunisation. On an unknown date, the patient received a dose dose of suspect Diphtheria, tetanus and acellular pertussis vaccine produced by unknown manufacturer (formulation, stremgth, dose and lot number not reported) via unknown route in unknown administration site for immunisation. On an unknown date the patient developed a serious had an anaphylactic reaction to the dtap shot because it had traces of dairy in it (anaphylactic reaction) (unknown latency) following the administration of Diphtheria and tetanus and acellular pertussis vaccine. This event was assessed as medically significant. Reporter was calling in regards to the COVID-19 vaccine. Her daughter got the first dose and the second dose and she was fine with both of those doses. The caller''s daughter is scheduled to get her booster dose tomorrow. The caller explains she called Pfizer yesterday and started this process. She was looking to see if there was any different in ingredients and inactive ingredients from her daughter''s Lots she had in June to the booster dose her daughter is scheduled to receive tomorrow. The caller states she sees on the website the inactive ingredients are based on the purple cap and gray cap, but only the purple is available. The gray cap is not released yet or something. The caller states her question today is based on her daughter''s anaphylaxis to dairy. She was looking to see if there was any different in ingredients and inactive ingredients from her daughter''s Lots she had in June to the booster dose her daughter is scheduled to receive tomorrow. The caller states she sees on the website the inactive ingredients are based on the purple cap and gray cap, but only the purple is available. The gray cap is not released yet or something. The caller states her question today is based on her daughter''s anaphylaxis to dairy. She looked on Pfizer''s allergen page and put in dairy and basically it says dairy products are used in the manufacturing process but are not expected to be in the final product. She always thought dairy wasn''t used in the manufacturing process, therefore there wouldn''t be any chance of dairy. The caller called the vaccine facility where her daughter will be getting her vaccine at to determine the Lot number because the caller wanted to find out if the Lot number of the booster dose was manufactured at the same place as the first two Lot numbers. Since her daughter did not have any reaction to the first two doses, she would feel better knowing if the Lot number for her booster dose was manufactured at the same place with the same quality control and cross-contamination process, things like that. Action taken with Diphtheria, tetanus and acellular pertussis vaccine was not applicable. It was not reported if the patient received a corrective treatment for the event. This case involves a 4 months old female patient who received IPV (VERO) [IPOL] that was not suitable for use post-excursion (product storage error) (poor quality product administered). The patient''s medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included a dose of DIPHTHERIA VACCINE TOXOID, PERTUSSIS VACCINE ACELLULAR, TETANUS VACCINE TOXOID (DTAP VACCINE); HIB VACCINE; PNEUMOCOCCAL VACCINE and ROTAVIRUS VACCINE (ROTAVIRUS VACCINE) for prophylactic vaccination. Write-up: fever; crying for 3 hours; Initial information regarding an unsolicited valid non-serious case was received from a consumer/non-health care professional via Medical Information (MI) (Reference number- 00899754) and transmitted to Sanofi on 15-Dec-2021. This case involves Child and unknown gender patient who had fever (pyrexia) and crying for 3 hours (crying) after receiving DIPHTHERIA-15/TETANUS/5 AC PERTUSSIS VACCINE [DAPTACEL]. The patient''s medical history, medical treatment(s), vaccination(s), concomitant medication(s) and family history were not provided. On an unknown date, the patient received a dose of suspect DIPHTHERIA-15/TETANUS/5 AC PERTUSSIS VACCINE (lot C576b5A, expiry: 28-Aug-2022) via unknown route in unknown administration site for prophylactic vaccination. On an unknown date the patient developed non-serious fever (pyrexia) and crying for 3 hours (crying) (unknown latency) following the administration of DIPHTHERIA-15/TETANUS/5 AC PERTUSSIS VACCINE. It was reported "Secretary states that a child had a reaction and was crying for two hours and also had fever after receiving DAPTACEL. The doctor wanted to know what else can be given other than Daptacel. Caller states that she would have the doctor call back with adverse event (AE) information and receive recommendation." No laboratory test was reported. It was not reported if the patient received a corrective treatment for the events. At time of reporting, outcome of the event was unknown. The reporter did not report the causality of events with suspect.
